options for birth

I discussed with my doctor what my options were to deliver my di/di boys. She said if baby A is head down in position these are my options. Id like your opinions/experiences. Long story short i've had all four of my children natural abd the thought of an epi terrifies me.

Option 1: get epidural when we arrive at hospital with no meds running thru. This will be used in case of an emergency c sec if baby b flips during labor.

Option 2: deliver with no epidural and in the case if baby b flips breech during labor they will place an epidural after baby a is delivered.

Option 3: not to get an epidural and if baby b flips after baby a's delivery and there is no time for an epidural I would be put to sleep.


All this info is overwhelming and scary to me. All my other singleton deliveries were calm fast and natrual. I used a yogo ball unil the time to push.

  • My answer is going to be influenced heavily by the birth I had with the twins, as a disclaimer.

    I'd have the epi placed with no meds. I had an emergency c/s and was put under and it blows. I would think that if the situation is emergent enough for a c/s, that there wouldnt be the time to place one, let the meds work, and then deliver. So I'd do the option that gives me the best chance to be awake for my babies' birth.

  • I had an epi line placed after I labored for a bit no meds ever ran threw and I had a quick easy delivery.

  • I would go with option 1.  You really should have the epidural placed before starting any of the delivery.  If you have to be put under general anesthesia, you and YH will both miss the delivery of Baby B and you'll always regret it.  Even if you deliver both babies vaginally, there's a possibility of complications after delivery that you may need an epidural for.  I had retained placentas, and even though I already had an IV in they quickly placed a second while I was in the OR so they could push enough drugs to numb and relax me during the manual removal.

    This! I couldn't agree more.  I was put under too, worst experience ever.  That being said, I ended up with two beautiful babies and would do it all over again if that was my
    only option for getting them out safely.  There are just so many variables when it comes to delivering multiples.  I hope yours is uneventful and complication free...but if things do take an unexpected turn, I'd rather be prepared with the epi already placed.  I know it's a lot of (very scary) information, but try not to dwell on it too much.  Wish you the best!

  • Option 1. I also had an emergency c section. Not only did my husband and I miss the birth, I didn't see my babies until the next day.
  • I would want the epi in place. What I had were none of those options; I got an epi when I was about 6-7 cm and did have meds. Baby B flipped back to breech at some point during labor/delivery so I did end up needing a breech extraction, so I was glad I had the epi.

  • I too was out all the way under for an emergency c-section, it was horrible!

    If these were my choices, I would go with #1. They can't guarantee that #2 is possible. There may not be enough time if something happens.
    Me: 37 DH: 40 TTC since 9/09
    #1 BFP 1/10/11; missed m/c discovered 7w5d
    IF Dx: Endo, hetero MTHFR mutation, poor morphology
    #1 IUI: 1/18/12 = BFN
    #1 IVF/ICSI 4/2/12 = 2 x 7-cell and 1 x 5-cell transferred (3dt) = BFP!!
    H was born at 41w2d on 12/29/12 - be still my heart!
    #2 IVF/ICSI 1/19/14 = 2 x 8 cells transferred (3dt) = BFP!! EDD 10/09/14
    M&W born at 37 weeks on 9/18/14 - I am the momma of 3 boys!!!

  • Just asked my OB about the epidural and having to be put to sleep. She said if baby B needs an emergency c-section I would have to be put to sleep anyways because there wouldn't be enough time to wait for the epidural medication to work (increase in dosage).

    Does this even make sense? I don't really know how to explain it. And I am tired.

  • My OB is willing to do vaginal if both twins are head down. If they are I will attempt it but I will get an epi just incase it's needed because I decide I want meds or I need a c section.

    With DD my water broke but no contractions so I needed pitocin. After several hours I was balled up in tears from the pain, apparently unconscious due to the IV pain meds they have me. I only remember the nurse telling DH I wasn't in active labor yet and so I immediately said I wanted an epi. It took forever. My contractions were so strong and close together I couldn't sit still. They had to poke me three times, I didn't really feel it though. Low and behold I was actually ready to push and the nurse was, for lack of a better word, an idiot for not checking me until after the epi was in place. So this time around I'm getting one early because I don't want to deal with that mess again.
