silent reflux: zantac versus prevacid

My LO (2 months) was diagnosed with silent reflux at a month. He was put on zantac, and the dose was recently evaluated/upped (more than a week ago). The issue is that his reflux is getting worse, not better - about an hour after feedings I see him cough up a little and grimace as if he his in pain. Many times he wakes up and begins crying. At times, he cries during feedings. Last night, he cried constantly for over an hour, and he has horrible painful gas. We know all the non-pharmaceutical tricks with reflux (positioning, burping, etc). We breastfeed and I will be doing an elimination diet (though I haven't seen any kind of a clear pattern and I've been paying attention). For those of you who have tried both medications, any thoughts on zantac versus prevacid? I guess prevacid is probably our next step, though the doctor has been hesitant to prescribe it due to off label status. I know they do different things and some children on are on both. Any input appreciated. We will be seeing the doctor later today and I'm hoping for a miracle - I hate seeing him in so much pain all the time.

    They work differently. Zantac is an H2 blocker and Prevacid is a proton pump inhibitor. My sister, who is a pharmacist, has told me that she's always seen more success with Prevacid than with Zantac, though Zantac is usually a first attempt medication as far as infant reflux goes. 

    I know the dosage varies A LOT. For example, the proper dosage for my DD is currently 0.7ml-1.8ml. That's a huge span. You said that his dosage was adjusted. Was it adjust per a new weight check or per the dosage recommendation for his 1m weight? If he hasn't been weighed since being 1m, it could be the change that would make a difference (he could need a dosage that is on the higher end of the scale for his current weight).
