I have to check with my insurance company, but I was told by a coworker that she got the Medela PIS and it was covered by the insurance co. Woohoo!!
I have Aetna...they don't cover it, unless:
For the period of time that a newborn is detained in the hospital
after the mother is discharged; breast pump rental is not considered
medically necessary once the newborn is discharged; or
For babies who have congenital anomalies that interfere with
feeding, breast pump rental is considered medically necessary for the
first month after discharge from the hospital.

I have Blue Cross Blue Shield...and an OB who has already stated that a pump will be "medically necessary" for me and will write a script
I plan on calling next week to find out for sure what the restrictions are though.
I am actually going to rent one from our local medicine shop. I am hesitant to buy, because with DD my milk never came in and she was losing weight. Therefore she was put on formula. I want to "try" to breastfeed with this DC, but I am not going to buy a pump until I know for sure.
I have the Medela PIS...LOVE LOVE LOVE IT!!!! when I was pregnant with DD I was seriously considering just getting a single electric, but then decided just to spend the extra money and get the medela. I am SOOOO glad we did that. It's so quick and handy...I was able to pump both sides at the same time in 10-15 min (6 oz. each)...and really holds up! it is DEFINITELY worth the money!!
I have the Medela Pump In Style Advance. I used it with my daughter, and I liked it as much as one can like a pump. This time around I will have that and then I am also going to get a hand pump (one of my girlfriends swears by hers).
I exclusively pumped with DS. After renting a hospital-grade one for a couple of months, I decided to buy a PISA. I was very happy with it. I got the tote bag but I think the backpack would work just fine also. I even had to pump in a plane one time and it was just fine (plenty of blankets, of course).
Check Lactationcare.com The have the PISA starting at $228. I got mine from an online site but the store was in NJ. I can't remember the name but the cost was similar.
Avent Isis Electrionic---I really loved it and had no problems with it. Although, what I really hate, is that they don't sell or make available replacement parts. Towards the end of my needing to use it, the parts got a little less snug-fitting. I don't know if I will use it or purchase a different model.
Re: which pump did you (or are you) going to buy?
