Hi, we live in a MCOL city which I think pays pretty well for childcare. We are going to have our 3rd baby soon and will start looking for a nanny. Our oldest will be in elementary school, the 4 year old will be in pre k program 3 full days/week. So, mostly the job would entail caring for the baby. What would be a fair hourly price? I know of people paying 13-15/hour for one infant in the area.
Also, if you have a nanny, how far ahead did you look for one? We would need one starting in August.

Totally depends on your area. Since your older 2 are not really full time, I might get comps based on 2 full time kids? I really can't give you an hourly rate.
As to timing, about 6-8 weeks out from when you need the nanny to start usually works.
I found our nanny on care.com, we absolutely LOVE her. As PP said, every area is different. When I created the 'job' on care.com, I answered a survey with number of kids and our zip code and it gave me the going rate for our family in our area. Since you may go that route to find your nanny, it could be worth it to check out the rates in your area.
It wouldn't hurt to start looking now, it took us about 2 months to find one with reviewing the profiles and setting up & conducing the interviews. We narrowed it down to 2 and on the 2nd meeting brought our kids in the room and watched how they interacted. Plus, even if you wouldn't need them to start until August, you'd probably want a few weeks transition to show them everything and have your kids get comfortable. You can clearly put in the job ad your timing. Good luck!
