Cervical Length Question - update

I am going for an ultrasound tomorrow to assess cervical length (internal will be done). At 12 weeks my cervical length was 2.8cm. My doctor did not seem concerned, however I pushed for an ultrasound because of early shortening with my first pregnancy (0.7cm @ 33weeks) and my mother having a history of late 2nd trimester loss b/c of incompetent cervix (I know technically there is no medical evidence to suggest a hereditary risk but it still freaks me out!). 

I have done lots of reading online but have not found a great resource for average or at risk cervical lengths. 

What was your cervical length at what week of your pregnancy that determined you were "high risk"? 

UPDATE: Thank you for all the responses ladies! Cervix measured 3.0cm at 18 weeks today. Looks like this may be my "normal" cervical length. I am happy and reassured everything will be okay. Wishing you all healthy pregnancies and babies!

  • With my first pregnancy, I had cervical incompetence and ended up being dilated 2cm with bulging membranes before knowing anything was going wrong.  Because of that, I had a preventative cerclage placed at 14w.  I was still probably around 4cm at that point, but given my history, they felt the cerclage was necessary (and I am grateful to have it!)

    I was getting weekly transvaginal ultrasounds up until a few weeks ago (I am 33w now).  My doctor was not concerned until I went below 2.5cm, as he told me anything above 2.5cm is considered normal.  I don't have anything to back that up, other than that's the guidance I was given!

  • I've been measuring around 3.8, and they told me that's considered very normal.

    I had a full term uneventful pregnancy prior to my current one. With this pregnancy, at my 12 week NT scan they noticed my cervix seemed short so did a transvaginal ultrasound and it measured 2.8 cm. The MFM (I was there for panorama screening and AMA) suggested we check again at 15 weeks. Everything looked 'good' at that appointment, although I don't know what the measurement was. Then at 18 or 19 weeks I and another appointment (anatomy scan) and they noticed the short cervix again. Back to a transvaginal ultrasound and I measured 1.7 cm with funneling. So I had an emergency cerclage placed. The thing that stinks is that if I just had the cerclage when it was 2.8 cm, it would probably be more stable. I'm also on progesterone suppositories. I would definitely ask to discuss the plan to monitor your cervical length, but also better safe than sorry and advocate for a cerclage given your family history and your shortening cervix with your first pregnancy....or at least progesterone suppositories. Did your first pregnancy go full term? I think my doc feels anything above 2.5 cm is good. Below and I will be asked to go on bed rest. Right now I'm supposed to be 'taking it easy'. Post-cerclage I've fluctuated from 2.7-4.2 cm. I have weekly checks until week 28 (next week thankfully!). Good luck to you and best wishes for a healthy pregnancy.
  • I was told they like to keep an eye on things if the length is below 3, but not really concerned until it goes below 2.5.

    I am high risk for other reasons but my MFM likes to check the length every 4 weeks anyway and mine has been measuring over 4 since week 12.
  • My cervix right now at 28 weeks is between 2.3-2.48, they can't really get a good picture of it, even after two ultrasounds. I was told 3+ was where I should be.

    I am on bed rest because of this + a positive FFN test + pre-term contractions, but my cervix hasn't changed. So therefore because of all those things I am technically high risk for pre-term labor.

    I'm not sure what the case would be if I just had the cervical shortening.

  • If you go to www.keepemcookin.com and click on the "Prevention and Treatment" section, it lists normal cervical lengths depending on how far along you are in your pregnancy.

  • My cervix measured 3.9 cm at 19 weeks and by 23 weeks had shortened to 1.9 cm and was put on bed rest. It's important to continue to check, especially as the baby starts to become heavy enough to to push on the cervix.
    Me:PCOS, fibroids DH: no issues 
    TTC for 3 years. IVF #1 in Jan 12 - severe OHSS; miscarriage at 11 weeks
    2 myomectomies and 6 hysteroscopies in 2012 and 2013.
    FET in Aug 2013 BFP

     
  • With my first son at 24 weeks I started having some tightening that wasn't normal and when I went in I was measuring 1.2cm. I was put on bed rest until 37 weeks and he came 2 days before my due date. It never got below 1cm and never had a positive FFN. With my second son at 19 weeks I measure 1.2cm again and was put on bed rest until 36 weeks and he was a repeat c-sec at 39 weeks. I never got below 1cm and never had a positive FFN. So far this pregnancy I am 21 weeks and last measured 4.1cm and so far so good. They don't seem concerned b/c both of my boys were full term. They didn't even want to check me at all but I asked to be checked last time. I'm hoping I don't end up on bed rest again b/c with two little boys running around it is going to be pretty hard.

  • My cervix has been monitored closely due to going into PTL and delivering DS 7.5 weeks early. They have no idea why he came early, but I had a kidney stone at 26w where they noticed the baby's head was low and my cervix had shortened to 2.5cm (from 4cm at my 20wk). A FFN came back negative, so I was sent on my way and never checked again.

    This time, my cervix has been monitored from the get-go. The highest measurement I got was a 3.5 at 16w. Every check I've had since then has been between 3.0-3.2cm, which my OB and MFM are happy with.

    I hope that they continue to monitor your cervix, as 3 is on the shorter side - but as a PP said 2.5 is when they begin to get concerned. Good luck! I hope that your cervix stays long and strong!
