STMs: follow up US. What to expect

At my A/S, the ultrasound found an EIF (bright spot on the heart) and slightly dilated kidneys on my little boy both of which are soft markers for downs. I went today to my normal appointment and I have a follow up US on the 28th to check on these markers. Any moms that had babies with markers and had to get another US to check them? What should I expect? Did the markers go away? I had the maternity 21 test and everything was good but I can't help but still be nervous. I know he doesn't have downs but now I worry maybe there will be an issue with his kidneys that caused the dilation.

  • The tech found an EIF on babies left ventrical during my 20 week ultrasound.  I went back a week after and had a second level two scan and it was still there but the specialist said it will go away on its own and that since the harmony came back with good results no need to worry.  Did you have the harmony, Mat21?

  • I'm not sure if this is the same when you say dilated kidneys, but my DS had moderate hydronephrosis found at his A/S. they followed up throughout pregnancy to make sure his amniotic fluid was enough (it was), gave him antibiotics at birth for about a week to prevent possible infection, checked for ureter/kidney reflux at 3-4 weeks, and followed up about every 4 months with ultrasounds. By the time he was about 18 months it completely cleared up and everything was fine. I guess hydronephrosis is one of the more common abnormal anatomy scan findings and is more prevalent for boys. Wishing you the best with your little one and pregnancy!
  • This one had slightly dialated kidneys at the a/s. They've gone down but are still a little more dilated than norm. I've been told not to worry about this as it's very common in boys and going down. And the numbers aren't too high.
    I don't know about the other, but the kidney thing was generally not a big deal as my dr explained it.

    Yes, I had the Mat21 and was negative for everything so I feel really good about that

    Thanks. At my A/S, his kidneys were measuring 4.1 and 4.2. 4 and under is normal. My doc said he hated to even mention it because we are talking about such a small number and US have a margin of error. When the tech measured them, they were both normal but when the doc looked for himself, he had a different number. The whole thing was just frustrating and worrisome.
  • DSs kidneys weren't picked up until 35w. He was born a week and half later with bilateral hydronephrosis. By 2.5 it was totally dissolved. BFF had found out her DD had it at 20w and it was dissolved by birth.

  • I had both of these things at our 20 week also... the kidneys were fine at our 28 and everyone involved says not to worry about the EIF (still there at 28) - it is really common and almost always meaningless.  

    Our OB explained it really well... he said maybe the heart spot ups your chances for Ds by 1.04%.  Fine, but do the femur lengths look good?  Because you multiple by 0.98.  Similarly there are many other markers that take your risk factor back down.  So even if you have a marker, all the other markers NOT being there should settle your mind a lot.  I hope I made his great insight make sense in my stumbling words....

  • Dilated kidneys also found at my anatomy scan. OB said we will check u/s again at 28 weeks (my next appt). She said it may still show up at 28 wk u/s and if it does, my pediatrician will follow up with baby. She said she sees almost an ultrasound a day with dilated kidneys (or hydronephrosis) and in her career has had maybe 4 or 5 babies who still had it at birth and maybe 1 or 2 of those who had to have surgery. And my LO is a boy which is more common. Good luck to you! I believe even if surgery is needed its nothing major. Believe it's outpatient. Still makes us nervous to have any abnormality with LO.
