Our new plan

So after 2 failed FETs, I requested a sit down with our RE to get her opinion and recommendations. It was scheduled for the end of March but they called on Wednesday and said that I could come in today - woo hoo!! I was happy to find out when we sat down that my RE brought our case up at a cycle review so I have input from other doctors in the practice as well.

So the plan is more testing to start. Both DH and I will have some additional blood work for a chromosonal analysis. Then I will also have a hysteroscopy and an endometrail biopsy to check for problems in my uterus (which all sounds very painful!). After that, we will move onto a natural FET and add on baby asprin and possibly progesterone depending on blood work. I was so happy when she suggested a natural FET!

The other thing that was brought up at the cycle review was transfering 2 embryos instead of just one, which I'm not completely opposed to, but I think we'd like to try one more FET before going that route. We're hoping they can work out the schedule so that I can do the testing and bw this cycle and then a natural FET next cycle. I should find out later today.

I feel pretty good about the plan. Any input on the testing? I'm nervous about the biopsy and hysteroscopy.

I have no input, but I am glad you had a good appointment and I am glad that Dr. D was proactive with your case. FX that things go smooth.

Can I ask what a natural FET entails?

Pretty much no meds! They'll bring me in about day 10 of my cycle for bw and an u/s to check my lining. And then I will have daily ovulation panels to pinpoint when I ovulate. I will transfer 6 days after a positive ovulation test. She's thinking maybe I'll have a better chance without all the meds. She said based on bw, they might add progesterone.

This seems genious to me! I am going to be asking Dr. N about this whenever I go back to consult on number 2. We are strictly MFI, it seems so much better to give it a shot (pun intended) without the hormones since I have no issues. Any idea why they don't start with this protocol??

Oh ccam, I'm so happy to hear this! I'm so glad your RE got more professional opinions, and I'm so glad you have a plan! No advice on the testing, but hopefully it's not too bad.

I think it's because they have more control over a medicated cycle, and they can tweak things based on your blood work. If someone's lining is thin, they can just continue on Lupron longer (I think) or increase your estrogen patches if needed. I won't be able to plan my transfer ahead of time like I was with a medicated cycle, but I'm ok with that. But since I have fairly regular cycles, she said I'm a good candidate for a natural FET... we shall see!

Yay for having a good plan!!!

I have had a hysteroscopy and endometrial biopsy. The bad news is that it did hurt. (The RE always says it caused discomfort but I know that is just code for it might hurt.) The good news is that the biopsy is so fast that you literally feel it for a second. Make sure you take Advil an hour before your appointment.

I also did a natural FET. It's the FET cycle that got me pregnant with K&A. The cycle was super easy. I was actually treating it like a break cycle mentally until I actually made it to transfer. Good luck!!!

Yay for a plan. My REs protocal included baby aspirin and progesterone for most patients as a precaution. Glad your RE is considering these.

I had an endometrial biopsy done over the summer and it was no big deal at all. Literally like a little more involved pap test, not painful at all. Prior to my IVF I had a hysterocopy and I didn't really find that painful either, perhaps just a little uncomfortable/crampy because they fill your uterus with saline. I actually found it kind of neat to get to see the inside of my uterus. So, don't freak out about those.

As for baby aspirin and progesterone, I always think if they can't hurt why not. I've taken them with all of my pregnancies since my IVF.

I am considering doing a natural FET as well, if my stupid cycles would cooperate (at this point they are all over the map from 22 days to 28 so not sure how that would work) so I'm hoping you have success.

Thanks for that @itsmevkb! I know everyone is different but that makes me feel a little better. She said they could put me out for it but that is so much more involved and would be much further out than doing it in office.

No ideas on the testing. Would they recommend PGD on your remaining frosties?

How many frosties do you have? The natural FET sounds great!

It was talked about but it wasn't something that the RE really recommended. She said since we had success the first time around for DS, she didn't think it was necessary. If we were doing a fresh cycle and could test those plus the frozen, she might suggest it. But to thaw, test and refreeze, it wouldn't be worth the cost.

We had 7 total so we have 5 left frozen in 4 tubes.

It sounds like you had a productive WTF appt and you have a good plan for moving forward. I haven't done any of the testing but probably have a hysteroscopy in my future too. My second saline sono is this Weds. I hope all tests are clear and a natural FET is all you need.

Thanks @dragonfly1226. I'm super nervous about being awake for these but in order to do it in the OR, I'd be pushed out too far. I scheduled the in-office hysteroscopy for next Monday. And I got a script for a Valium

@ccam - I promise it will be okay. Valium is good. Also remember to take Advil an hour before. After it is all done you might just be a little crampy. I drove home from mine alone both times... But I guess you probably won't want to drive on Valium.

good luck it's great you have a plan! As far as the hysteroscopy goes I had it done awake with no meds and it wasn't bad at all.
