High-Risk Pregnancy

HELLP - Second pregnancy - specialist?

Hi all,

I got a BFP yesterday and had a consultation with my OB today. I had HELLP syndrome with my first pregnancy and I am considering seeking a consultation with a specialist. My OB feels confident in her ability to help me manage our risks this time, but because of the possibilities, I would feel more comfortable at least discussing things with someone who has more specific knowledge. Unfortunately, there's not a huge amount of research behind HELLP, although some scientists lump it in with Pre-E/Ecclampsia (but it's more complicated IMO because it involves liver and kidney function).

Anyone here know what field I could start looking into? More specifically - I didn't know if a high-risk OB would be who I need to consult with or perhaps a renal/liver specialist?

As a side note, I had undiagnosed Hashimoto's thyroiditis (basically a thyroid autoimmune disorder) prior to DS. Some research indicates that autoimmune disorders such as Hashimoto's have a link. I am under the care of an endochrinologist and have requested an appointment for Wednesday of next week.

PG#1 - 3rd cycle BFP. Team Green. HELLP syndrome @ 34 weeks.
Later diagnosed with Hashimoto's Thyroiditis, possible link to HELLP.

PG#2 M/C 3/14 - Surprise BFP 2/13. Beta's doubled every 52 hours from 3w5d-5w5d
Viable pregnancy scan at 5w5d; 2nd u/s showed 2 days of growth in 7 but a HB of 120
3rd u/s on 3/10/14 had no HB and baby had only grown 7 days over 14
D&C 3/17/14 - complications - DX Retroflexed uterus, multiple tears to cervix

  • First, congratulations!  I had Acute Fatty Liver, which doctors *believe* is related to HELLP. I know how frustrating it can be to find someone who knows anything about it! I am also having a second pregnancy right now. Since HELLP is a condition specific to pregnancy, I think your best bet would be a MFM doctor. Our MFM specialist had never specifically worked on a case like ours, but he is monitoring my liver and kidney function throughout this pregnancy.

  • A friend of mine had HELLP syndrome with her st pregnancy and had an immediate c section where she was sent out of town because of blood transfusion. 

    However, she switched doctor's for her second pregnancy and did not see a specialist. And actually had a successful, med free, VBAC with no complications about 9 months ago with her 2nd!

  • I had HELLP with my 1st. I was high risk then as well for a pre existing heart issue. This time around my high risk status is higher bc of HELLP (delivered at 35 weeks), pre-e with my 1st pregnancy, gd with my 1st pregnancy, my age, and my pre existing heart stuff. Makes for an interesting ride. My high risk OB (that is who you should seek out) told me that my chances of HELLP with the 2nd is lower, esp if its with same father. In any case I'm being very closely monitored by both OB and high risk OB.

    Just got sent for baseline 24 hour urine and blood work and from what I understand, that will happen several times throughout pregnancy.

  • I'm nervous about HELLP with this one as I had it with the first. I'm hoping my OB will monitor me closer this time.
