MFM preconception visit question XP from Preemies

My son was born at 31 weeks due to severe pre-e with neurological complications.  I called my ob the other day to ask about baby aspirin and see if there is anything I can do pre-conception to decrease my risk of pre-e... I knew the answer was nothing more than to make sure I'm as healthy as possible at this point.  What I didn't expect was the nurse telling me that she talked to a doctor and that they don't recommend baby aspirin or anything during pregnancy because the cause of pre-e is unknown so there is no reason to take something that is not guaranteed to help.  I love my ob office and want to stick with them but now I'm thinking I want to see a MFM in addition.  I don't know if the nurse was just speaking on her own or if she really did talk to someone. there are a lot of doctors at the office so I will never know who it would have been. Even if the MFM just says the same thing I think I will feel better.  the MFM that saw me when I was in the hospital is in network and is the chief of the MFM department at our hospital.  He's well respected and is taking new patients. I know that there is a lot of research on ways to decrease risk of pre-e during pregnancy and I want to do anything that will keep my baby and myself healthy.  I will also be over 35 when I deliver if that matters.  

Did anyone see an MFM for a pre-conception visit? Do you think seeing him piss off my ob or could it be offensive to them? I do not need a referral for insurance purposes.  Both my husband and I remember him saying that we should come see him if we wanted to before we had another baby but my ob wasn't a part of that discussion.

  • I see no reason your ob would feel upset about that, mine works together with the MFM. He should be happy that you are trying your best to be responsible with the cards you have been dealt :)

  • I did not see my MFM for a pre-conception visit, but my OB suggested that I should.  I meant to, but got a BFP earlier than expected and things just went from there.  In my situation, my OB and MFM communicate regularly and seem to have a good working relationship, which is part of why the OB recommended I do the pre-conception visit.  I don't see why your OB would have a problem with it, and it would be a little concerning to me if he WAS upset.  I guess it can't hurt to try to speak to your OB directly (not the nurse) beforehand, but I would be hopeful that they'd support your request to see the MFM.

  • Thank you! I have no reason to think they would be but I'm a people please by nature and worry! The MFM and ob are in the same hospital and he's the chief so I'm sure they work together frequently.

  • My cardiologist recommended I see one a few years ago when I didnt want to take birth control. I thought it was very helpful and lots of good information. When I did find out I was pregnant (2 years later) I wasnt freaking out about my heart condition like I probably would have if I hadnt gone. I ended up switching to the MFM as my main OB because I liked her so much!
  • Thank you! KFerrell I think that is part of it... I think if I see the MFM now I will freak out less when I am pregnant.... which will help with the blood pressure I hope! I called this morning and the woman who schedules new patients won't be in until next week so I will talk to her then and make my appointment!

  • I saw an MFM before getting pregnant to figure out how to mitigate my risks.  I'm surprised you didn't get referred to one with your first delivery being complicated.  If your OB doesn't like it, that's their problem.

  • I also delivered at 31 weeks (well, 30w 6d) due to pre-e and placental abruption.  We were told to meet with MFM before considering another pregnancy, which I figured they would tell us not to do, but they told us to go for it.  As soon as I got a BFP I had to go in for a viability ultrasound and as soon as we saw the heartbeat I was started on Lovenox (40mg, once daily) and baby aspirin.  I also take fish oil and vitamin D in addition to my prenatal vitamin.  Even though they don't know if these things will help prevent pre-e, I was told that they are no downsides so it make sense to try it.  I had an appointment this morning and we are 12w and looking good!  The hope is to make it to 39w for a repeat c-section...I am hoping we can do it!  Good luck to you and never worry about hurting someones feelings.  This is your life and you need to be in control of it!
  • I had my daughter at 31.0 due to severe pre e. I tried to make an appt with my now MFM prior to my current pregnancy but they wouldn't see me until I already had a BFP. I'm currently 30.3 (!!!) with #2. I didn't see my MFM until week 20 with this pregnancy but see them now every 3-4 weeks due to previous pre e and SUA diagnosis. So far, so great! I'm on aspirin and Bp this morning was 111/66! Good luck to you!

  • I saw a MFM before my first pregnancy bc my cardiac specialist and regular OB wanted some guidance. He listened, gave me some thoughts, and suggestions.

    My first pregnancy went fairly well. I did develop GD but that could have been age related (I was 38 and delivered at 39). I delivered at 35 weeks due to severe pre-e with HELLP. I did see both MFM and regular OB alternating weeks for my entire pregnancy. They worked very closely together.

    I am now pregnant with my 2nd. A little more high risk than last time as I still have cardiac issues, plus I'm 40 now, and I now have history of pre-e with HELLP and GD.

    Both MFM and OB are again working together. Both are cautious and happy we are covering all bases. Both are trying their best to keep pre-e and other complications at bay.

    I think you need to see the MFM and I don't think your regular OB will mind!

  • I saw an OB and a MFM doctor during my pregnancy and they worked really well together.  I don't think it would anger your OB to seek an opinion from MFM.
  • I called the MFM and I am waiting for an appt (we've had crazy problems with snow/ice/power losses lately) I had to call my dr.'s office to get my records released.  The nurse seemed to give me some attitude about why I was seeing an MFM and I explained to her that I was told at the hospital that I can see someone if I would like before I try again.  It really bothered me. I liked my OBs but if I feel any pushback when I go in to see them in a few months I'm switching.  They should be thankful I'm trying to be proactive so that their jobs should be easier!
