I started with one formula bottle a day. I thought about mixing, but decided to see if my LO would just drink formula on its own. Not mixing is easier, so if he would drink it that would be best for us. I feel lucky he cooperated and enjoyed his formula.
DH and I are traveling out of the country for 3 weeks next month and leaving the girls with family, so we're pretty much forced to go from EBF to formula. I was really stressing about it since LO had had maybe 3 bottles of breast milk in her life and had always refused formula, but I offered her formula (really warm) in a sippy cup and she loved it! She likes to suck on DD #1's sippies, so maybe that was a good introduction. Turns out she does just as well with bottles (we found a kind she really likes), so she's getting between 1 and 3 bottles of formula on top of breast feeding every day to ease her into it.

@libberluru it's worth a shot starting with the easy solution. If your LO doesn't take to it and you want to change tactics later, no problem. I had to remind myself of this when deciding what to do.
My supply is dipping too (on purpose), so we've been doing straight up formula (2oz) occasionally to introduce it to him. We haven't had to go straight to formula yet because of my freezer stash, but we will in about a month or so.
I just started adding formula this weekend. I added 1 ounce of formula to 5oz of BM to start with. I just wanted to be extra careful with his sensitive tummy. He's been doing good so ill keep increasing. I do agree with pp that mixing BM with formulas kinda a pain in the ass. Once I know I found a formula that works I won't be mixing anymore. My goal is to have him drink mostly formula during the day mixed with freezer stash and nurse morning and night. I just want to ditch the pump. GL
H did a cocktail for a little bit. I then added a bottle a week. I started with one of formula and then went to two and so on. I had some set backs when Lily got sick, but, after 2 months, we are now down to nursing at night, MOTN, and morning. She doesn't want to sit still long enough to nurse during the day now. The cocktail allows baby to get used to the formula. H just gradually lessened the BM in the bottles as we went.
I cut my pumping sessions to twice a day and supplemented whatever I didn't pump with formula. Then I started cutting his bottles in half, mixing bm and formula for all bottles. I did that for a few weeks. Then I replaced one feeding, then two, then three with formula. I never pumped on the weekends, I would just skip nursing and offer a full formula bottle. He fought it for a couple weeks, but we've been completely weaned now for almost 2 weeks. The whole process took a little over a month.

+1 and it's been great, I still pump 3x when I work 8-5 but B gets 2 bottles of bm and 1 formula. I am building a stash back and less stressed about supply. Good luck!
