odds of multiples with treatment?

I'm still new to the infertility stuff but I have been looking into options and most seem to have a high chance of multiples.  So how does one go about choosing a treatment plan.  I'm ok with twins but triplets and more seems like a really bad idea, does it vary from cycle to cycle what the risks are? Do most Dr.s stop treatment if it looks like you have the possibility of to many and just wait for the next cycle?

  • I have twins run in my family, and did IVF w 2 transferred each cycle (except last- only had one left). Dd started as twins and twin didn't make it. Multiples higher risk, but not a guarantee. The craziest thing to happen would be two embies transferred and splitting, but there is really only a 30 or so % chance for one to take. If worried too much, you can ask they only transfer one at a time. Of course this is IVF I am talking about- if doing clomid or IUI I don't know much about those- but I am sure someone does on here!

  • Yes, they will cancel a cycle if you have too many potential follies. Just like if you do IVF, they will ONLY put back 2, unless you are over 40, to reduce the risk of HOM, but nowadays, a lot of people elect of do an ESET, depending on their clinic success rates. So there are a lot of ways to avoid these situations. Octomom was the result of an ignorant Dr.

  • You always have the option to bail out of a cycle if there's too many follies/high chance of multiples.

  • I remember octo mom as she had hers while I was pregnant the first time.  I also have a cousin on my mom side with 12 year old triplets (identical girls and a boy) they are natural.  I saw how hard it was for them with three, they came early after mom was on bedrest forever then to raise three was hard.  
    I figure we will start with IUI and meds since insurance covers parts of it and it can be done closer.  We have an appointment tomorrow so I want to be ready to talk to the Dr. We are not even sure where we will start.

    I had three follicles this past cycle and it was my option to continue or not. I chose to go for it because one was smaller than the other two.
    Edit: I forgot to add that this cycle was medicated only.

    @KC1212 Yeah I guess bailing out is always an option, though hard to do with how much it costs.

  • @blackvelvet well I always took the gamble and it never worked anyway :(
    I wouldn't worry about twins. Triplets and up scare me though. It's not the amount of work it would take to raise them that I find scary but all the potential health risks that worry me.

  • In all my minimal stim cycles (clomid) I had 2 follicles, only ever had 1 bfp and that was a singleton (ended in mmc). If you go with injects you run the risk of a higher instance of overstimulation, which can lead to canceled cycles if you aren't being closely monitored. When you move onto IVF, like @mrscastillo and @mamarazzi007 said, depending on your clinic, 2 will be the most you can transfer. Fwiw we are planning on transferring two.

    Two DDs 10/08 and 08/10, no primary IF
    TTC #3 since 10/2011 - dx unexplained/weak ovulation
     3 BFN clomid + TI cycles, 5 BFN clomid/gonal f IUIs, 1 mmc IUI
    2/19/2014 IVF #1 Unexpected low E2 (oversuppressed) -> increased to max doses = 3 or 4 follicles, converting to IUI
    BFFP Saw 1 beautiful heartbeat at 6w6d, follow up u/s at 9w showed mmc. Eff this.
    NTNP 5/2014-9/2014, OPKs and TI 10/2014 - 1/2015. 
    RPL testing all normal, AFC, AMH, and FSH all normal. 
    IVF 1.2 1/22/2014 natural cycle start, AFC 28, 300 gonal f/150menopur. 
    ER 2/3/15 14R 8M 3F w/ICSI Day 5 transfer on 2/8/15 of one "Grade A+" blast and have TWO frosties!

  • what everyone has said, when starting with a doctor i would ask them how they handle high order multiples and what precautions they have in place to avoid creating a high order multiple pregnancy. my RE is very cautious, they will not proceed with a cycle if there are more than 3 mature looking follies. the cycle we got KU with our DS, we went for our last ultrasouns beforevthe IUI and we had two definitely big mature follies and then one that was seriously borderline, she went through what that meant and it was up to us whether to go ahead or not. So while doctotrs should be careful, do your homework and make your own informed decisions during the process. good luck

    TTC#1 May 2009- July 2010 on our own with no luck

    Started with RE in August 2010, dx with unexplained IF and then finally our 3rd IUI cycle using Follistim and Trigger resulted in our wonderful little man.  Born 12/2/11

    TTC#2 Never really prevented, but were careful early on as Dr. reccomended

    Surprise BFP 12/16/13, started progesterone immediately as first numbers came back low, but betas were good.  Progesterone wasnt enough. Natural MC 12/24/13.

    Back with RE as of January 2014...

    5/27/14- Chemical Pregnancy :(

    April 2015 IVF#1

    5/13/15- BFP, please stick LO!

    Oh and I'm a major Harry Potter Nerd :)

  • My RE gave me the % chance of twins and trips with a Clomid/injects and timed intercourse/IUI cycle. I believe they used their own data to develop the percentages, but maybe it was national data they pulled from. Either way, ask your office, they should be able to give you an idea of what the chances are.

  • I've heard the chances of multiples are higher with medicated IUI's than it is with IVF because there is less control. Personally, when we were going through it all before I conceived my son, I was praying for twins. I hoped we would get a two for one, so to speak and then we wouldn't have to worry about IF anymore. But during the pregnancy and in the early days, I was glad to only have the one. But now that I'm back here, I wouldn't mind twins. It's a pipe dream, though. I really just want another baby!
