I *think* I belong here? (Intro)

Hi Ladies!

I've been a member of TTCAL for over a year now.  Have had 3 consecutive losses, and no longer have any hope of having things happen "naturally".  Now that we are turning to a Fertility Clinic 8 hrs away to help conceive, I'm going to need a lot more help than just the ladies on TTCAL can provide.  Can I join you here?

About Me:  I am a couple of months away from turning 35.  DH and I have been together 14 years, but married for almost 8.  I was diagnosed with Endometriosis when I was 20, and we knew all along getting pregnant could be difficult.  We stopped BCP's just before the wedding, and ended up getting pregnant the first cycle off them!   DS is a wonderful boy.  He's an "old soul" and practically raises himself.  We were scared to have another child, because we didn't think we could handle a difficult baby after having such an easy one.  When DS started Kindergarten though, we decided to give him a sibling.  6 months after we started trying I had a missed miscarriage.  Since that time I've had 2 more miscarriages.  RPL testing has turned up insulin resistance, estrogen dominance and PCOS.  

DS is now approaching 7, and it breaks my heart that we waited so long to TFAS, when we should have known it might not be easy.  We were just lulled into complacency since the Endo didn't seem to effect getting pregnant the first time.  

Anyway, we have no choice but to see a Fertility Specialist out of town, due to there not one available anywhere near us.  The nearest clinic is an 8 hr drive away, with the rest being 9/10 hrs away.  As part of our pre-screening before our consult, we've repeated our bloodwork, and DH did a sperm analysis as well.  Here are the numbers:

Volume= 6.0
Appearance= Normal
Concentration= 61
Motillity= 82
Motility Progressive= 65
(All good right?)

Normal Forms= 0
Abnormal Forms= 100
Immature Forms=3
(Yeah, not so good)

My consult isn't until March, so I'll have to wonder what this means for our treatment plan until then.  In the meantime, DH is going to start taking a few more vitamins, and trying to get more anti-oxidants in his diet.  I just don't know what else to do.  We really didn't want to do IVF due to the long distance we'll have to travel, but I'm thinking these numbers make it more likely?  

Anyone else been in the same position?

tl;dr.... TFAS (DS 6).  DH and I almost 35.  3 consecutive m/c's in 2 years.  PCOS, Endo, Insulin-Resistance, Long/Irregular Cycles, Estrogen Dominance, Possible MFI due to Abnormal Numbers.  Travelling long-distance for fertility treatments. Looking for support as I begin this process :-)

  • Welcome- I'm sorry for your troubles. We are battling poor morphology, too. DH is at about 3.5 - 4%. His other numbers are all in normal range, but they're right on that borderline so who knows what that means. He has bilateral varicoceles but the doctor said repairing those won't help morphology much or at all.

  • Sorry you're going through something similar @annibes.  I thought Varicoceles did effect sperm? That's interesting your doctor says surgery won't help.  

    Just to be on the safe side (and since we have to wait a couple months anyway), I'm sending DH to get checked our by a local Urologist.  Wait times for appointments are long where I live, and I don't want to waste anymore time just in case some sort of surgery is necessary.

    I'm in a bit of disagreement with the doctor, but I'm not a urologist so I have to take my own advice with a grain of salt, you know? Varicocelectomy can help with sperm count, I think, but morphology doesn't seem to improve with repair. My argument is that if we up his overall numbers, then 3-4% of a high count could be just as good as 15% of a low or borderline count. But eh.

    We are pretty much doing what you're doing- getting as much info as we can. It's a slow process and that's frustrating when time is such a factor!

  • Welcome I know you from TTCAL.  Sounds like you are in a similar situation with a few of us.  I understand a long drive to an RE ours will be at least 4 hours away (and three mountain passes so make that 6 in the winter).

    So we are severe mfi too, and IVF is usually the route for that. Seems like you are in right place! Hubs had varicocele repair and the stats on it are 33% see improvement, 33% see reduction, and about 33% see no change. We were in last group of no real change. His went from 1 million with bad motility and morphology to 3 million with slightly better morphology and motility. Still suckage. We are gearing up for fresh IVF cycle in March/ April.
    8 hours away?!?!?! Holy crap!!! But the things we do for our babies right? Welcome and hope your stay is short and sweet! A lot if great women on here!

     3 BFN clomid + TI cycles, 5 BFN clomid/gonal f IUIs, 1 mmc IUI
    2/19/2014 IVF #1 Unexpected low E2 (oversuppressed) -> increased to max doses = 3 or 4 follicles, converting to IUI
    BFFP Saw 1 beautiful heartbeat at 6w6d, follow up u/s at 9w showed mmc. Eff this.
    NTNP 5/2014-9/2014, OPKs and TI 10/2014 - 1/2015. 
    RPL testing all normal, AFC, AMH, and FSH all normal. 
    IVF 1.2 1/22/2014 natural cycle start, AFC 28, 300 gonal f/150menopur. 
    ER 2/3/15 14R 8M 3F w/ICSI Day 5 transfer on 2/8/15 of one "Grade A+" blast and have TWO frosties!

  • Thanks for the welcome ladies!  I'm a little scared now that we're moving on to assisted reproduction.  I have NO idea what to expect, and the entire idea is just so overwhelming right now. 

    @Mommarazzi007 -  You nailed it....I am indeed a Dr. Who fan!  My 6yo is too.  We went to the Disneyland Halloween Party and he was dressed as Dr. Who.  DH didn't think anyone would know who he was.  He was wrong...DS got a lot of attention that night :P

  • My DH had a vericocele repair and the improvement was only really seen in the area of motility. The clomid he took helped with overall count. Right now we are facing low count and motility again. Our urologist would like him to take the clomid again for 3 months or 90 days, as that is how long it takes to see a change in SA parameters. Then he will have another consultation, this will be March for us, to see if a vericocele repair is needed again.

    MFI - DH had varicocele repair and took Clomid to get DD b. 02/2010

    TTC #2 since 6/2010
    10/2012 DH diagnosed with Epilepsy
    A few failed IUIs summer 2012 and 2013. 
    DH taking clomid and waiting to see if he needs another vericocele repair. 
    Hoping for a 2015 baby or babies.

    Awesome! For DD's 1st Halloween I was Pond, dd was Vincent Van Gogh (also my fav artist) and hubs was Doctor :) good times!!!

  • I know zilch about Male Factor Infertility, but I know plenty about IVF.  I would think it would be possible to partner with an ob clinic closer to you so that you could do all your monitoring appointments/ultrasounds during a cycle closer to home (for us it was every other day for 14 days) and then make the big drive for egg retrievals and transfers.  Certainly worth asking them about.  

    If you watch "The Little Couple" they seem to have done it that way.  Yes, I'm basing some ideas off tv, silly I know.

    Trying for #2 with IVF after damage done in a D&C left me with one functioning (but too scarred) ovary and tube to get pregnant safely without assistance. It's a really, really long story, but that is the gist of it.  We have one daughter (whose c-section birth started this roller coaster) born in 2012 after a miscarriage in 2011. 
    Failed 1st attempt at IVF (Fresh ET day 3, 1 frozen embryo saved) in December 2013.  
    Failed 2nd attempt at IVF (Fresh ET day 5, 2 frozen embryos saved) in February 2014.
