IUI for MFI (morphology) ?

Hey Ladies,
I am gearing up for my RE appt on Tuesday where we will develop at medication/treatment plan.  We have been working with him since Sept and only done diagnostics thus far so I am really ready to hear what he recommends as far as treatment.  My guess from family/friends experiences and the internet is that we will start with IUI?  So far our diagnosis is borderline low morph (5% in Sept and 9% in Dec)/unexplained.  I am just curious to know what your thoughts/experiences are on IUI success for morphology/unexplained.  I don't have much data but a strong gut feeling that IUI is just not going to do much for us :(  I understand how IUI would be beneficial if you needed to trigger ovulation or if there was a motility issue it would get the sperm closer to the tubes.  For morphology what would the rational be for IUI and how much does it really increase our chances of conception?

  • IUI may not be first step, what are his numbers? If below, what 5, 15 mil? (Help me ladies my hubs has never been above 3) then IVF w ICSI would be recommended route, not enough for IUI to really work. That being said, if only 5% are good morphology, but those are a good number, whatever high millions, IUI could still be option because of sheer numbers. It's a numbers game, after all motility and morphology are considered.

  • I agree with PP, from what my RE explained morphology wasn't as big of a deal if the count is really high, or at least normal. For instance my DH's morph was around 5% I think, but his count was 95mil so the RE wasn't concerned. Also I believe some RE's use different criteria, Kruger model vs others which changes the percentages around.

  • DH's morphology was only 1% Kruger. He was dxed with mild MFI. But he had really good count and good motility. Our first RE recommended IUI. We did 1 IUI and it failed. The second month we chose to skip IUI and do TI and we got pregnant. We are now seeing a new RE (due to a move) and he said he would never recommend IUI for low morphology. He recommended TI, which is what we are doing. I agree that morphology is the least important 'defect' in sperm and IUI is not necessary unless there is a low sperm count or motility issue. Also, IUI is not really fun. DH felt so uncomfortable with the whole thing and his count on the day of the IUI was really bad. He put it down to performance anxiety and he thought that was the reason I didn't get KU that cycle! For us, it has been much better to do TI since it is more relaxing for both DH and I.
    I'm 29. DH is 29. 
    TTC #2 since 1/14
    DX= PCOS and Annovulatory
    Cycle #1- Failed IUI 5/12
    Cycle #2- Injectibles and TI- BFP 7/13/12. Due 3/2/13

  • I somehow missed this post  8-} MH's first SA over a year ago came back with 3.5% strict morph, but high count (I think 70mil that time). My RE also said the high count can outweigh the low morph. MH just had a repeat SA and his strict was up to 10%, so it really can change. In regards to IUI, we've done 6 and 1 resulted in a pregnancy - I also have crap eggs though, and this last IUI was pretty much thrown out the window when I got a fever/stomach bug the day after, so 1/5 I guess. So if his count is high and you're "normal", then I'd say go for a couple IUIs, especially if insurance covers it. IUIs will also involve washing the sperm, which typically leads to an even better sample that is used. 

    Two DDs 10/08 and 08/10, no primary IF
    TTC #3 since 10/2011 - dx unexplained/weak ovulation
     3 BFN clomid + TI cycles, 5 BFN clomid/gonal f IUIs, 1 mmc IUI
    2/19/2014 IVF #1 Unexpected low E2 (oversuppressed) -> increased to max doses = 3 or 4 follicles, converting to IUI
    BFFP Saw 1 beautiful heartbeat at 6w6d, follow up u/s at 9w showed mmc. Eff this.
    NTNP 5/2014-9/2014, OPKs and TI 10/2014 - 1/2015. 
    RPL testing all normal, AFC, AMH, and FSH all normal. 
    IVF 1.2 1/22/2014 natural cycle start, AFC 28, 300 gonal f/150menopur. 
    ER 2/3/15 14R 8M 3F w/ICSI Day 5 transfer on 2/8/15 of one "Grade A+" blast and have TWO frosties!
