Spit-up/reflux worse with BF- any ideas?

Our DS has mild reflux- just the typical 2 month baby reflux.  He isn't on any medication and usually it doesn't bother him.  He eats directly from the breast usually 4-7 times a day and gets 2-3 bottles depending on my work schedule, etc.  Usually his reflux is just him spitting up a little and swallowing it again-occasionally he spits up a teaspoon or less.

For the last week or so he has been spitting up a lot more-- like tablespoons/ounces (and last night almost a whole feeding) when he is BF, and usually doesn't spit up at all when he is bottle fed breast milk.  He is usually BF in the morning (side-lying in bed for the first feeding and another in the glider before work), and then from dinner until bedtime, including overnight (basically BF when I am home, bottle during work hours).  I stopped nursing side-lying in the morning since I figured being close to flat wasn't good for reflux, but it still doesn't seem to be helping.

I've been burping him a ton and trying to keep him upright for 20 minutes after feeding.  Do you ladies have any other ideas to fix this or reasons why it could be happening?  I don't want to pump and give him more bottles than I already do, but also hate to see him spitting up/throwing up every time he is BF.

  • I noticed DS spit up peaked around 4 months. He is ebf.

  • Is he a happy spitter?  If so, it's a laundry issue, not a medical one. Some babies spit up more than others.

Do you have an oversupply or overactive letdown?
He is happy during the day when he spits up- so that didn't worry me at all, but at night he isn't sleeping well because of it.  I do have a slight oversupply- I pump about 4 ounces more than he needs a day- usually first thing in the morning from the side he didn't nurse on overnight.

  • I'm trying to pump less now that I have a freezer stash for when I'm at work.  But when I return to work I'll have to pump a couple times everyday.
