Breech Twin A at 30 weeks (and a couple of position questions for BTDT moms)

Hi, all you trusty twin mamas.

Baby A (our girl) has been footling breech during the past month. The most recent ultrasound was Friday at 30 weeks. My OB will happily perform a vaginal delivery as long as Baby A is head down and not too much smaller than Baby B, and I strongly prefer a vaginal delivery if possible. Before this last appointment, he had been telling me that there was still plenty of time for her to turn, but on Friday he said that he would be "surprised" at this point. The majority of the movement I feel seems to be near my bladder/cervix/pelvic floor, so I know that (at least for now) she is still feet down, kicking away down there. 

If your babies switched positions in the third trimester, how late did this happen? With singletons, this is a common time for them to turn vertex from breech, but obviously with twins there is less space. What do you think? Do we still have a chance at a flip and a vaginal delivery?

  • My baby B flipped from breech to vertex sometime between 30-32w. He had more room though. Baby A flipped breech at 19w and stayed that way. At every u/s the tech would comment "wow, baby a is really wedged down in there" so I didn't have much hope of him flipping. He didn't and I had a c/s. I would have def gone vag if it had been an option.

  • My baby A was still breech at my 32 week scan but we found that he flipped head down at my 35 week scan.

    When the flipped- B dropped into the birth canal (head down) and technically became baby A from that point. I ended up delivering vaginally- B (the new A) normally and the second baby breech- it was a perfect delivery!

    Basically, what I'm saying is that anything can happen lol. These babies have a plan of their own, and you still have time for things to change

    This is what I was hoping would have happened - that when B flipped to vertex, he would've displaced A and then give me a chance at vag.

  • Both my boys were head down until 30 weeks, when A flipped to breech. Unfortunately he never went back :-\ B flipped literally every few days, he somehow had plenty of room ;) I was hoping he might muscle his brother out of the way and become my new A, but it didn't happen. It's harder for twin A to flip past 30 weeks, but there are ladies here that have had it happen as late as 36 weeks. There's definitely hope! But it still would behoove you to start reading up on what to expect from a C section delivery. Knowledge of both delivery types is power.

    You could also check out the exercises at spinningbabies.com (they even have a twin section), or look into seeing a chiropractor who practices the Webster technique.

  • My B flipped head up to head down 3 times within my 10 minute vaginal delivery with my midwife and once from head down to breech just before I started pushing! I never felt him flip either ( he flipped almost every time I had an u/s in the days leading up to delivery too) at this was at 34 weeks. You would be amazed at how much they can actually move it there! Seriously I never felt him flip!

  • I'd trust what your OB said about chances. Twins can flip late in the game, but it really depends on how they're positioned to start and how much room they have left. My OB would let me attempt vaginal delivery so long as A was vertex and that's what I was going for. You can always schedule the c/s and see if it ends up being needed.

    My babies were "stacked" on top of each other (think Legos) and completely transverse...automatic c/s situation. They did not move from these positions after 27wks. At 33wks they told me there was no way they'd move given how little room was left (A was wedged in so tight) so we scheduled my c/s for 37wks. They did a u/s minutes before my c/s just to make sure and they hadn't budged. Unrelated side note: I'm now very glad they didn't move because B had his cord wrapped 4x around his neck. If I had attempted vaginal, things could have gone very badly...or ended up as a double whammy at best.
