So everyone at work keeps asking me if DS is ready to be a big brother and I have always responded that I don't think he really understands and probably wont until LO is here. (for those that dont remember, DS is a little over 2 and speech delayed and in special instruction and speech therapy 2x per week, so his expressive communication is not that great - improving but still not where he probably should be).
Well over the weekend we had a birthday party to go to for friends of ours little girl who just turned 1. DH had DS at the store to pick out a present for her and DS refused to pick out a gift until he got DH to agree to buy this little bedtime bear for the baby. He kept pointing to it and when DH asked him if that is what he wanted for the present for the little girls birthday present he said very clearly and firmly "No. For baby." DH clarified that he wanted it for his baby sister and he got a big smile and said yes. Then once it was in the cart, he finally picked out a present for the little girl.

can I ask a personal question? Does he have just a speech delay or have they diagnosed something else? As of now they have only found a verbal speech delay in my son, he does speech once a week and two hour preschool three times a week. Good luck and thanks for sharing
At his 18 month well visit it was found that he had a double ear infection that I did not know about - he literally had NO SYMPTOMS at all! I felt like mom of the year on that one. After 3 rounds of different antibiotics, the fluid was still not clearing up so I requested to be seen by and ENT and discuss tubes. He had his hearing tested and they said he had a substantial hearing loss just from the amount of fluid in his ears and since he had no symptoms of the ear infections, it was likely they had been going on for a while. He had the tubes placed in both ears shortly after.
Meanwhile, I had contacted Early Intervention to get him evaluated for services, which he did meet and they started him on special instruction once per week. At his six month review of Early Intervention services (around thanksgiving) they added the speech therapists once a week becasue though he was starting to increase his vocab, it was not as fast as we had been expecting and he was mispronouncing several things. He has had his hearing tested since the tubes were put in and his hearing is fine now so they are considering a diagnosis of either phonological processing disorder or childhood apraxia of speech. Both seem similar and he is still pretty young for a diagnosis of either - part of what the speech therapist is doing is further evaluating him.
Right now we are still working on increasing the number of words he can say, and the clarity of them, and working on combining words into two and three word sentences.

Your post makes me miss working with my kiddos. Worked outpatient pediatrics for almost 4 years and now work with geriatric, which I love as well! One of my all time favorite patients was a little fellow with apraxia. It was the absolute worst case I have ever seen - poor thing spoke zero consonant sounds. I believe I worked with him twice a week for a bit over a year and he left speaking in complete sentences with 100% intelligibility! Your little guy will catch up soon!
