Micro-TESE procedure - logistics

H is having micro-TESE on Friday at surgery center by urologist. RE's lab just called with instructions. They include me driving the specimen in approved kit that I will pick up that morning to the lab within 45 minutes of the dr. extracting the specimen. I'm not sure how I thought the specimen would get to the lab, but I didn't realized I'd be the one doing it.

Well the surgi center to the lab is 22 miles - that will take me 30 minutes in no traffic or incidents. What if there is a back-up of some sort, or God forbid an accident? Not a lot of wiggle room there (actually none because I'm assuming there will be some kind of process in the OR after the specimen comes out before the kit is handed to me).

AND I just checked the weather and there is a potential for rain/snow Friday.

Ugh. I am freaking out about this but obviously can't discuss this with H, because he shouldn't have to hear my silly anxiety about driving a freaking sperm kit to the lab when he is worried about the actual operation.

Sigh. Just a vent I guess.

Re: Micro-TESE procedure - logistics

  • When my husband had his TESE, they said to get it there as quickly as I could while being safe. There's nothing you'll be able to do if it gets there after 45 minutes and it shouldn't hurt anything if it's a little longer than 45 minutes. Can you call the andrology lab at your RE and ask more questions about the actual timeline? Good luck!

  • Just wanted to wish you and your DH luck on Friday. I know how stressful it is. We insisted on using fresh sperm so DH had his TESE while I was having my ER. Needless to say there was a lot of running around since we both couldn't drive and the sperm needed to travel 20 miles ASAP. I'm really glad I didn't have to do the running around. Don't stress too much- if you're doing IVF with ICSI you only need one sperm per egg- if a few die off because you're stuck in traffic, there should still be plenty left.

I am thinking of asking my mother-in-law to come with me even though that will be painful. Just to have a back-up and someone to drive it over with me.

  • It's kind of unclear. They identified sperm in the OR, but when andrology went to QC a sample, they only found one and it was non-motile. They also froze a small portion and in the thaw test they found nothing.

However the urologist and RE both believe there may be enough in there if there was one in the QC. But we will need to do another one timed with retrieval because they're not all that confident apparently.

We have an appt with the urologist next week to discuss further. It's still a little confusing for me. If its good then why isn't it good ENOUGH? And if it isn't, how we do know a fresh procedure will be better?

  • I can totally relate.. hopefully they will have better luck with fresh and this frozen sample could be used for a back up in case nothing else happens. Wishing you lots of luck!
