Intro and question about twins measuring 5 wks apart

Hi! My name is Michelle and I am 28 weeks with twin boys. I've been a big lurker but this is my first post. I had an ultrasound and appt with my dr today and he said that twin b is measuring 2 weeks ahead and twin a is measuring 3 weeks behind, so they are 5 weeks apart. He said it wasn't horrible but it is concerning so I have to call a perinatal dr (?) tomorrow. And of course I have no interest in waiting so I was wondering if anyone has experienced this? My dr is very relaxed about everything so it's hard to tell what is a big deal and what isn't. Thanks :)

  • Size discordance is mostly measured in percentages if comparing the twins (as they have done with my identical modi twins), but if yours are fraternal there are fewer and different risks involved since the placentas would be separate.
    As far as measurements of ahead/behind, these are based on a statistical normal curve, which come with variation for any baby.

    As for mine, Baby A is right on track, measuring in the 36th percentile for gestational age while Baby B is measuring in the less than 2nd percentile and about 4 weeks behind. Anything under the 10th percentile is cause for concern with my MFM.
    Since they share a placenta as modi twins, there is a 36% discordance in size with anything over 20% being cause for concern.

    As for your pair, it really depends on if they are monochorionic or dichorionic that will determine risk level considering genetic differences, unequal placental sharing, TTTS (which is a whole other set of measurements and only concerns monochorionic pairs), etc. Also, not every ultrasound machine is created equal, so I would wait and see what the perinatologist says tomorrow. If they are anything like mine, you're in fantastic hands!

  • Wow that is quite a difference. Di-di twin boys, I'm assuming?

    I know size difference is less concerning with di-di twins, but I'd say it's a good thing to have a perinatologist check out to rule out cord flow and placenta issues, etc.

  • Yes sorry they are di-di. Thank you guys! I'm not the most patient person when it comes to all this.. But I'm sure the perinatologist will be able to explain it better than my dr did because I'm super confused.
