PPROM with water break at 26 weeks Update

I think I just need to actually type out the information we received today to help me process!!

At 26 weeks I had pprom with a water break and I have been in the hospital ever since. Our plan has been to deliver between 32-34 weeks.

The past week I have been leaking more (I still wouldn't say a lot!) and my crotch hurts horribly!!!

Today we had an appointment with our perinatologist. He was shocked at how low our little girl is and said she can't get any lower without coming out. She is the one that is ruptured and her fluid levels are low and lower than what they were. I seen him measure anywhere between 1.5-2 cm only! The girl is measuring 3 lbs 5 oz and the boy is 3 lbs 9 oz.

My OBGYN stopped in and informed me he talked with our perinatologist and our plan has changed. We are now delivering at 32 weeks via C-section. At this point I won't be receiving the steroid booster shot as they don't think it is necessary. I will receive some magnesium on the way to the OR. My OBGYN and perinatologist both said they wouldn't be surprised if I go into labor before 32 weeks and are starting to seem less optimistic! My OBGYN asked if 32 weeks was like 8 days away and I laughed and said no...13 days! He then said well 30 weeks 1 day is better than 26.

Things really just got VERY real.....

  • Hang in there! I delivered at 32 after a PPROM, and both girls are home and doing well now. The twin whose sac did rupture did better in the nicu with breathing because the rupture sent distress signals to her to start maturing faster. Her sister didn't have that benefit, so she was in the nicu about a week longer.

    Even if you deliver tomorrow, they are going to do so much better and be in the nicu for so much less time because of how long you've kept them in there, so great job.

    Have you been having contractions at all?

    Married 8/2008. IVF with PGD March 2013.
    3/22 ER: 25R, 20M, 15F. 9 genetically normal, and 3 survived to Day 5
    3/27 ET: transferred 1 embryo, beta 9dp5dt=163, 12dp5dt=639
    4/25 1st ultrasound at 7 weeks = identical twins with heartbeats?!!!

    Can I ask how long your NICU stay was? That is interesting that the one who ruptured for you did better but it makes sense! I have also heard that generally speaking, girls do better than boys too!

    The baby with the rupture was born at 4 lbs and was in the nicu just shy of 4 weeks (27 days). The smaller baby was 3 lbs 6 oz and in the nicu for 5 weeks (35 days), and did go home on a bit of oxygen, but weaned off that in a couple of weeks at home.

    They are gaining weight like champs now and at 10 weeks (2 weeks adjusted), they each weigh around 8 lbs. and are gaining over an oz a day.

  • The fact that you've made it this long since you PPROMed is great!  You're doing an awesome job of keeping those little ones in.  I made it 4 weeks 1 day after I PPROMed before my ruptured water broke entirely and I went in to labor.  My doctors were thrilled I'd lasted that long.  Good luck to you. I hope you make it to 32 weeks!!

  • I hope your little girl can hold steady on her fluid a little longer. You've done a great job keeping everyone inside this long. T&P's you can get as close to 32 weeks as possible!

    You have done so well and come so far! You should  be proud. You are in good hands and  your babies will have a wonderful team to help them transition to life on the outside. Big hugs.

  • You've come so far, 32 weeks is great from where you started! I hope you can it make, hang in there! I'd still request a booster round of steroids at 32 weeks. When were your last shots? I wonder why they think they won't benefit.

  • You have done so well keeping them in as long as you have! Hopefully they are content to stay put a little longer. It was my baby A that broke her water and when she was born, she came out screaming! She was the smallest of the three (2lbs 2oz, 27 weeks) and is a little superstar so far. Best of luck!!

  • That's great you made it so far! I delivered at 32weeks 1 day due to pre e and hellp syndrome. I didn't have time to get a booster or steroids. My Dr assured me at that gestational age they would be ok without it (not that we had a choice). We are 20 days out and both are still in NICU and one has had a really rough road. All of the NICU Drs have said if I was given steroids some of his problems probably wouldn't have occurred. And he was my bigger twin at 4lbs 10oz so please ask your dr about the shots again! GL to you! :)
