Goodbye gift for Daycare provider

This is my son's last week at his current daycare.  We have had some scheduling challenges and have decided to switch daycares.  She's been caring for my son since he was 3 months old so I'd like to give her a nice (personal) gift. Do you have any suggestions? 
Me: 36 (Endo) DH: 39 (Azoo)
5 DIUI - BFN
IVF#1 - BFP - AJ 7/12
FET#1 - BFP Due 7/24/14

    I vote for a gift card of some kind (spa, store, restaurant etc). I would also do a small food basket with some of her favorite treats or holiday treats.

    So, in her Christmas card I added a $50 spa gift card, and I purchased a 3 picture collage frame and put a recent picture of my son on one side and a picture of the two of them together on the other side, and in the middle I put my son's hand print with a note saying "Thank you for taking such great care of me.  I love you.  Andrew".  When she saw it she started crying. :)  I felt so sad... it was like breaking up with someone...
