? for those with non verbal kids

It occurred to me yesterday while out shopping with DS1 (3 years old, and non verbal) that if he ever got separated from me while at the mall or grocery store he wouldn't be able to give his name or my name to someone to find me. 

I'd like to get something made to either pin to his jacket/shirt or clip to the zipper of his jacket with his name and my cell number. Does anyone have something similar to this that could point me into the right direction as to where to go to get something like this made, or what all it should say? TIA

    There are also a lot of ID wrist band products for runners that may work depending on the likelihood that your son would remove it himself.
