WTF APPT AND NEXT STEP...DOR Dx

Today we went to the WTF appt. Our RE said she was happy with the cycle and it was just an egg issue. I have poor AMH 0.61, but I am only 35 (haha like 35 is young... not in the IF world) but none the less, she thinks that one cycle could produce not so good eggs and the next might do the trick. Also, we discussed using an egg donor. We looked through the donor book and found a couple that we maybe would pick. The embryologist called me and said that she went back through and looked at our embryos and she doesn't think that giving up hope on my eggs would be the wise choice. She said that with my age and my response (I had 6 mature eggs retrieved) that I should try again. She said sometimes a different protocol will change the outcome.

I am not sure how to proceed. It would be twice as much if we went with a donor but the doctor said our chance of pregnancy would increase..UGH. At this point I would even want to look into embryo adoption but my hubby said no. I just want a baby to hold and love. It would be our child.

we are 100% OOP. We still have a 10K loan we are paying from the first cycle. I told my DH that if we can save 8k, and sell our boat we should try again in March with my eggs one more time. If it doesn't work we move to donor eggs which we could even do in a year or so since we wouldn't be so pressured because of my DOR.

I think I am just overall sad and scared to go through this whole thing again and face the disappointment again.

You ladies with DOR what are your thoughts? how many cycles have you gone through or are willing to go through before you go to donor?

  • I'm really sorry about your cycle. As for me, I had such bad DOR that I didn't even respond to max stims (600iu) injected right into my muscle. So the decision was made for me, and in a lot of ways I am grateful for that. Decision making can be the hardest part of this whole thing.

    I wanted to tell you that if you wanted to also post this question over on the Third Party Reproduction board, there are lots of women who use DE over there that can tell you of their experiences. I wish you luck in whatever you decide!

  • I was 37 when we started and never got more than 3 eggs. Two times we only got 2 eggs and once we had no transfer because nothing made it to day 2. My DOR was very severe (AMH is 0.16 and my FSH clocked in at over 24. That was almost 2 years ago). I was "fortunate" though to have insurance so we did 4 rounds of IVF with my eggs and 2 more with donor eggs. We're now paying OOP for a guarantee program at RBA using frozen eggs. I was ready after 2 IVFs but my DH wasn't. Then we chose a terrible clinic for the first 2 attempts with DE. Just know that DE is not always the quick fix RE's sell it to be. Do your research and you will probably want to travel. Don't just stay with your current RE "just because".

  • @Mrs.McIrish Thank you.. I am so sorry you have DOR also. My first AMH came back at 0.16 and my second was 0.61. I also had one fsh 12 and another 7.2. I have switched RE once and will definitely take your advise of looking around. It cant hurt. how did you pick your DE from the first two transfer's?  what made you move to frozen? the cost?

  • My first AMH was 0.92 at 37.5 with FSH at 12.1. 6 months later, the AMH dropped to .3 and FSH to 17.3. 6 months later, 0.16 and FSH over 24. I haven't had anything tested since we moved to DE. Part of me wishes we did DE sooner as I may have had a baby by now instead of nothing after 3.5 years. It's so tough to know what to do when there isn't a money tree in the backyard! With my first two DE tries, I still had insurance for the medical part. I found a clinic that offered lower cost cycles where you got a certain number of eggs (shared cycle). I knew their stats were lower than national statistics but I had also heard they worked with insurance so I gambled that I'd get lucky despite their stats and not have to pay much money OOP. I was wrong and they were really unethical with my 2nd try(long story). We got terrible embryos both times. We tried to be CFNBC from February until I had a crying breakdown in Sept. and we decided to do a guarantee program. I wanted frozen eggs because I had a bad experience where my 2nd donor backed out the week before her ER. It's risky to trust people to do this and I just didn't have it in me to go through again. I also couldn't travel with a day's notice for a donor's ER with a new job and can plan ahead more easily with frozen as the eggs are there waiting for me. It's not for everyone but it works for us this time. I'm matched with a donor and my ET is supposed to be 1/11.

    Did they suggest how they might change up the protocol? I agree that switching up a protocol can make all the difference. I also see that you got 3 frozen in the first round- that seems pretty good for DOR!!
    In my experience, my first protocol wasn't the right one for me. I had pretty much lost hope since it was aggressive and I felt that if that didn't work, nothing would. They changed it up and it wound up being a really successful cycle for me. 
    As PP said, if you are up for it financially and emotionally, I would probably give my eggs one more try and then go back to the drawing board if it doesn't work. There is just so much money involved either way, that I hate that you have to go through this and have to make this type of a decision. Good luck with whatever you decide to do.

  • I am not a great responder and have chromosomal issues on top of borderline levels. We are planning one more fresh IVF. We had only 1 embryo last time, so we're hoping for at least that on our next cycle. If it doesn't work, then we are planning to moving on to DE.

    For our upcoming cycle, we are changing things up quite a bit. My first was a straightforward natural start antagonist. Next time we are taking a blanket approach- estrogen priming lupron flare (using gonal f, menopur, lupron and hGH during stims, ugh!)

    I hope you and DH can come to a decision that is comfortable for you, and that whatever path you choose brings you a take home baby!

  • Congrats on being able to move on to the next step. I also have DOR (it was 0.77 but now 1.1) and I think we will pretty soon be looking into donor eggs. I know I have a hard time making decisions and once I make this one there will be no going back. As my therapist says, I'm going to have to mourn the fact that I won't have a baby that is genetically mine. I'm doing 2 IUIs as a last ditch effort to say I've tried everything. 4 traditional IVFs - 1 free IVF via a study. We also have an issue with injectables and no sperm- which I know is not your case. 
    Just wondering, have you tried IUIs with injectables? If you can get 3 eggs and have no sperm issues, maybe that is an option for you to get another try in while still attempting to save up for donor eggs.

  • I'm DOR, but we are also MFI, so we are currently discussing how many more rounds if IVF before adoption. We can comfortably afford one more, but couldn't do a third right after that (need to save) and that's one of the things we are discussing at our WTF tomorrow. We are also 100% OOP and the meds are killing us since we are at max stims. Like someone else said, I want a baby, so we are really weighing trying for the third round vs. getting the adoption process started ASAP.

  • I am new to the DOR club, but I would move forward with your own eggs again. You had a decent response with the first one, so a change in protocol could be just what you need. I know I will be so hopeful for my first IVF, but I am going into this knowing that we will likely need multiple rounds for IVF to work. Take some comfort in that your RE will have learned from the first round and have some ideas on what to tweak to make the next round better. fX for you!

  • Thank you everyone, your all so sweet. I actually slept better last night just having a resolve to the last chapter that the IVF# 1 is over. I think we have decided to move forward with our eggs again. I am going to find out what protocol she wants to try next and I am going to research that, Also I am going to research other clinics and possibly change. I think that if I try everything over the next couple months to increase our egg quality we might get a good response, going to start DHEA again while we wait. I was talking to a girl last night who is my age and has an AMH lower than me and she only had 3 eggs retrieved and she just had her little girl. I wonder if my progesterone spiking before egg retrieval had something to do with it, also after the FET my estrogen was 818 and 7 days later it was under 200. I want to know if any of these fluctuations would cause a BFN. I know that the timing of everything has to be perfect.

    I feel comfortable giving it one more go with my eggs, and then we will move to donor. Praying and thinking positive thoughts for each of you. <3

     I'm sorry you are having to make such a hard decision- I was torn up about this decision after our first 2 IVFs failed. My AMH was .3 for what it's worth. I think it is a very difficult and personal decision so I think it depends what you and YH feel comfortable with. For us, we wanted to try all our options (which meant a microdose Lupron flare and EPP protocol) before moving on. I agree with PPs that protocol seems to make a huge difference and really did for me. I also did DHEA, coq10, acupuncture, green smoothies, and a high protein diet. Who knows if any of those helped! Good luck and hang in there!

     I'm sorry you are having to make such a hard decision- I was torn up about this decision after our first 2 IVFs failed. My AMH was .3 for what it's worth. I think it is a very difficult and personal decision so I think it depends what you and YH feel comfortable with. For us, we wanted to try all our options (which meant a microdose Lupron flare and EPP protocol) before moving on. I agree with PPs that protocol seems to make a huge difference and really did for me. I also did DHEA, coq10, acupuncture, green smoothies, and a high protein diet. Who knows if any of those helped! Good luck and hang in there!
    Thank you, Yes we are going to use these next couple months to try and improve egg quality. I was on the flare protocol before.  I will go back to acupunture, add the DHEA again, I drink green smoothies but I do need to increase my protein. I am vegetarian. So I will start doing protein powder to help with that. I am so glad you got your BFP! congrats!

    10-29 baseline appt scheduled
    11-20 scheduled date of FET #1
    12-2 BFN :(

    3-1-14 moving on to the next step. Switched RE. going to plan 3 IUI cycles while saving for IVF#2

    May 2014- IUI#2 BFN
    June 2014 IUI#3 BFN
