GD question for you mama's!

I'm popping over from march 14 for a general question for ya'll.

How do they determine who needs to be on insulin vs just a diet restriction with GD?

  • My doctor has me check my blood sugar 4 times a day.  When I first wake up (fasting) and again an hour after each meal.  If my numbers are within the range he provided then it is considered controlled by the diet.  If any averages are over the range provided (they average the weeks morning, lunch, and evening numbers separately) then we will discuss taking oral meds or insulin to control it.

  • I was started on insulin right away, based solely on my fasting number being too high. I take it once before bed everyday. It has been upped once, and I have an appointment today to followup with MFM, so we shall see if they want to make any changes at this point.

    Our Angel was born sleeping at 20 weeks due to IC.

  • My dietician says that it couldve been controlled by diet, but my mfm put me on an oral med anyways...its worked so...my only high numbers were after dinner.

  • What PP said, you start out with just diet and see if you can stay under the range they want you in.  They give you a couple weeks because sometimes it can be difficult to figure out which foods do and don't work at first.  The first dose that people usually end up on is a before dinner dose to help control your fasting numbers.  After about a week, they still weren't thrilled with my other numbers so I was also put on a before breakfast dose, and over several weeks my dosage has been increased a little for both doses.

    It's been well over a month though since my dose was increased, but I suspect it will need increasing at least once more once I am in third tri.

October 2012 Metformin 1500 mg= ovulation on CD34 BFP#2 11/14/12 9DPO, EDD 7/26/13, DX Gestational Diabetes @14 wks, our angel born sleeping 3/24/13 @ 22wks 2d. BFP #3 7/4/13 8DPO EDD 3/22/14, DX Gestational Diabetes @14 wks. started insulin @16 wks.  Our rainbow, born 3/19/14 @ 39wks 6d., we're so in love!

  • Depends on numbers I guess. I'm on glyburide 1.25 once a day, but as your pregnancy continues your placenta can continue to raise your numbers, so you might have to up dosage or switch meds. Generally you would start with diet control, but again that depends on your original testing numbers.
  • Mine isn't bc of diet (I barely can eat -hyperemesis- and eat well when I do), but bc of twins. I started on Metformin due to PCOS before pgcy and all BS was bad. Thurs I was put on insulin (Humulin R & N) twice a day. Had to up dosage of R already bc it wasn't enough. I feel so much better as of today.
  • I also started insulin early, just based on my last pregnancy they knew I would need it. I also need to test 4x day to determine if my insulin dosage is working. It took a few adjustments to get it right.
