Ugh. Daycare rant. I dont even know what to say....

So, I rarely trust anyone else to give my children medicine. But, as I mentioned in a previous message earlier, DS has a double ear infection and is on an antibiotic. In order to slowly move up the time we give it to him every day (it was 6:30 last night....), I sent it to daycare today and requested that they give him 1/2 tsp at 4:00. No biggie. They have a medication sheet and give medicine, when needed, all the time. I had a little bit of uneasiness about it, but have always been very comfortable with those girls (heck, one of them was our first non-family babysitter!). I got a call at 4:05 saying she gave him 1 1/2 tsp instead of the prescribed 1/2 tsp. Although the bottle says 1/2 tsp, and my note says 1/2 tsp, there was some bad handwriting on the dry erase board of daily activities and she thought it said 1 1/2.

I am so upset. I immediately called the doctor and just talked to the nurse. She said there was really nothing we could do, but will talk to the doctor about what specifically I should look for, if anything. I will definitely have a convesation with the daycare director regarding medication safety, and double checking doses with another person. Heck, as an RN, there are steps we have to take to ensure our patients are getting the correct medication, in the right dose, at the right time, etc. Ugh. Anyone have experience with an antibiotic overdose? What to look for? I have a feeling his poor belly will be tore up tonight. :-( Any other measures you would take with the daycare?

  • I wouldn't worry about an OD, it's a tsp more, one time.

    I know it is just a tsp more, but heck, his prescribed dose is only 1/2 tsp....so its 2x more than his body needs.

  • Talk to the doc, but I honestly don't think you have much to worry about with this overdose.  I think the bacteria have more to worry about than him ;)  Keep an eye on him, of course, but don't get too concerned.

    I'd be ticked, but to me it matters more how they address this--ie, maybe a whiteboard isn't the best place for notes on medications or whatever.  If they don't find a good solution, that's when I'd bust out the crankymom.

  • It totally sucks to have something like that happen whe you aren't there. I would also talk to the daycare nursery about medication safety, but I wouldn't place any blame on any of the caregivers since it seems like they were doing there job correctly.

  • Is it a once a day med? Why are you having daycare give it? Leo has had plenty of ear infections (just got tubes so fx no more) and I have never had daycare give his meds, even when they were twice a day.

    Sorry for the mix up. I wouldn't bring meds to daycare unless there was no other option.

  • To the post who asked why daycare was giving it....it is a once a day dosing, but his first dose was given last night as soon as we got it, at 6:30 pm. I want to avoid giving it close to bedtime, so I was trying to push back the dosing time an hour or 2 every day, which is why I asked daycare to give it today at 4:00.

    I just picked him up from school and he's acting perfectly fine. Pediatrician called back and I do need to report it to Poison control.

  • Without knowing which antibiotic it is, I can tell you it may cause some mild nausea.  No other symptoms expected from a one-time dose of this amount.  PM me if you have any other questions!

  • That's definitely frustrating. Even if it's once a day though you're okay to not wait 24 hours between doses here and there so I'd just start giving it to him in the morning before you leave for DC. Sorry, hope you're little guy feels okay tonight!

  • I'm so sorry that happened.  I would be really upset and worried about it too.  I don't have any advice for you but I hope he's ok and doesn't have any bad reactions from it.  Hopefully they change their process for giving medicine and have someone double check the dosage so this doesn't happen again.  Good luck!

  • Ugg I know its scary but I wouldn't worry. The pharmacy screwed up one of Alices prescriptions and she was triple dosed on her acid meds. It just made her tummy upset. I think everything will be fine, Im sorry you have to deal with this :(

    Poison control is not a reporting agency, so definitely wouldn't need to call for that reason.  But they're always there if reassurance is needed or to double check toxic doses and find out what should be done!  And again, you can PM me if you need any more info!

    That's what I mean. A pediatrician saying to "report it to poison control" just seems.... we'll go with the word strange.

    I would definitely be pissed that they screwed up the dosage. Luckily it seems like LO will be ok.

     I agree with @Cheenomae. Just give it in the morning; it's ok for it not be a 24 hr spacing.

    Sorry for the post and run ladies. Our evenings are quite chaotic after work!
    Not exactly sure the reasoning behind the suggestion to report it to Poison Control, but that's who she told me to call. To be honest though, I didn't call them.

    @Pharmer78, the medication is Cefdinir.

    @Cheenomae, if his last dose was at 4pm today (Tuesday), can I wait longer than 24 hours so I can give him the next dose Thursday morning? Heck, he had 3 days worth today.... But I know it doesn't quite work like that.

    I do plan to follow up with daycare to see what they have in place to make sure it doesn't happen again. The teacher made an honest mistake as she thought "the board" said 1 1/2 tsp. They need to read specific prescription labels and not a handwritten board.

  • I'd be disappointed if this happened to ERy because they should be using the sheet you filled out and the directions on the bottle, not a white board. I would just be up front, asking them what they are going to do to make sure it doesn't happen again. If they can answer with a reasonable plan, then I'd be fine with it.

    'call poison control' is a cover your ass thing for the provider.

    and most of hte time, poison control will say "go get checked at the ER if you are concerned but they are fine". and then the parents come to the ER, and as per usual, they are fine.

    And DS is, indeed, perfectly fine. :-)
