Moms of boys, what kind of diapers are you using? We've always used Pampers but right now I think he's between a size 3 and 4, and suddenly it is blow-out city... we've never had that issue before, and in the last week I've totally destroyed 3 outfits. Boo.
James is in size 9 or 12 month clothing and he's a big boy -- although now for the life of me I cannot remember his most recent stats. 23 lbs and 29 inches, maybe? I know he was in the 92nd %ile for height and weight.
Re: Another diaper question...
My guy is 16# 8oz and 27.25"long and I'm using Huggies or BRU band size 3 during the day and Pampers size 3 at night. So far we haven't had a blow out, but have come close when he is in his pooperoo (aka jumperoo).
We use Huggies during the day, they moderately contain a blow out. If it's a bad one, it's still up to his neck and I don't think anything other than cloth can remedy that. Makes me nervous for our first visit to Santa, having to stand in line for a long time. I'm praying he'll poop before we leave the house (but you KNOW he won't! He's Totally gonna crap all over the white onesie I'm putting him in!!)
We double diaper with the Pampers 12 hr at nite and that keeps the leaks in from LO sleeping on his tummy.

In my experience, the Swaddles are horrible at holding in/catching poop. Give the Baby Dry or Cruisers a try if you are a Pampers fan. Cruisers are Pampers' "high end" diaper for bigger babies (much like the Swaddlers for infants) and were the next step for more active babies before they made Swaddlers in the bigger sizes.
ETA: Also, can't hurt to double-check basics (time to move up a size? done up tight enough? gussets on the side un-tucked so poo can't follow them out?) but I bet you did that already
ETA2: Just checked the Pampers website...size 4 fits 22 lbs and up on all 3 types of diaper
