DD is about 16 months actual (more like 14 months adjusted), and I still find myself giving her a few bottles a day even though she's primarily on solids now. Partly because she still loves her ba-ba, but I also find that I still feel more comfortable with the fact that she's getting all those extra vitamins on top of what she eats at meal times. How old was your preemie when you finally stopped giving formula (or breast milk if that applies to you) completely! When did you make a complete switch to "regular" milk?

We had feeding issues and relied on formula as the main source of nutrition until about 15 months. We then transitioned to WCM, since she was eating more solids.
We switched at 12 months, per my pedi. I also did a transition to WCM by adding formula to her bottles. She was transition to a sippy cup at 13 months old.
Dd has a milk allergy so we went back and forth until about 14 months actual 12 adjusted. It took awhile for her bottom to clear up from the rash caused by her milk allergy so even though we were cleared to try lactose free (which she drinks now) we didn't want to try anything new until she had fully healed from the reaction to cows milk.

Our DS was always on the skinny side (he's just naturally long and thin) so our Ped had us start weaning to whole milk at 12 months adjusted, but just starting to sub one sippy or bottle a day with whole milk. Every week or so we'd add another as whole milk instead of formula. I'd say he was completely switched over by the time he was 16 or 17 months actual.
We stopped at 1 year actual about 9.5 months adjusted. That's when we transitioned to sippy cups instead of bottles (probably more like 13 months to cut out the before bed one). But at 1 year our boys were eating a decent amount of food with good variety so that and a multi-vitamin was enough for me and our pedi.
We started transitioning to WCM at 12 actual. It took about 2-3 weeks to make the switch. She is 16 months and down to 1 bottle before bed. We switched to milk first and then started reducing the bottles.
Adalyn was on a bottle until about 15 or 16 months--she kept having some weight gain issues at 12-month appt and the pedi didn't want her to start WCM just yet since it was less calories (We were giving her Similac Advance, but making it have more calories per ounce). We would give her WCM in the sippy, and mix WCM and formula in the bottles. We stopped the bottles around 16 months and did straight sippy, which was just WCM. She didn't even really seem to notice!
