dashed hopes - I don't understand

My RE just called and my 3 day BW is worse than expected. In July my AMH was .16 and now it is .09 and my FSH is 21. I hadn't had my FSH tested before on day 3 so I don't have anything to compare that to. But how can my AMH change so fast.

I just don't understand. In September we used Bravelle and clomid and I had 2 follicles: 15 and 23 in size on my left ovary. We tried IUI and this is also when we discovered my left fallopian tube was blocked and the IUI failed.

My RE said we can go ahead and try the injections and see what happens on Monday when they do blood work but he doesn't expect I'll produce much. So tomorrow morning I start 150 menopur and 300 gonal f in the evening.

I just don't understand this.

  • My RE told me that your AMH can change up or down. Mine was lower before dd was born and now with starting ivf #2 it went back up. Also have you asked about dhea and cq10 some have reported success with both egg quantity and quality using them.

  • I feel like after 2yrs of this I should be an expert, but I don't really understand what the AMH number means, and I don't think I've ever had it tested. 

    @breeanne1974 I'm sorry you've been thrown a curve ball, and really hope the meds do the trick!

    Two DDs 10/08 and 08/10, no primary IF
    TTC #3 since 10/2011 - dx unexplained/weak ovulation
     3 BFN clomid + TI cycles, 5 BFN clomid/gonal f IUIs, 1 mmc IUI
    2/19/2014 IVF #1 Unexpected low E2 (oversuppressed) -> increased to max doses = 3 or 4 follicles, converting to IUI
    BFFP Saw 1 beautiful heartbeat at 6w6d, follow up u/s at 9w showed mmc. Eff this.
    NTNP 5/2014-9/2014, OPKs and TI 10/2014 - 1/2015. 
    RPL testing all normal, AFC, AMH, and FSH all normal. 
    IVF 1.2 1/22/2014 natural cycle start, AFC 28, 300 gonal f/150menopur. 
    ER 2/3/15 14R 8M 3F w/ICSI Day 5 transfer on 2/8/15 of one "Grade A+" blast and have TWO frosties!

  • I'm just devastated. I feel like I should be medicated for my anxiety with all this. So glad I'm working from home because I'm an absolute mess.

    Is it still possible the stim meds would work? I really felt like my RE was not optimistic at all and it would end up being a wasted effort. I wonder if he is just being really conservative about the expectation.

  • I'm sorry, that's such a bummer.  I don't know much about low or high AMH/FSH, but I would think that your RE would have pulled a new/more aggressive protocol for you.  I hope he's just being conservative and the higher dose brings great results!

  • I'm so sorry...IF is so frustrating & unfair! I would ask your RE if he can up your stim dosage. Also, you may want to think about changing doctors if you don't have good results this cycle. I don't know that much about it, but I believe the FSH is one of the first tests that should be run. I also wonder why he didn't do an HSG test on you prior to your IUI. There are probably valid reasons for this, but in my experience these tests were run prior to determining a treatment plan/protocol.

    I hope you begin responding well & this cycle works out for you. Good luck!

    This is my second RE - my first RE did the HSG and discovered the blocked tube and we didn't want to waste the cycle so we did Bravelle and IUI. He sent me to this new RE and I walked in his office and said "I have no time to waste. Not even one month." So I guess we are finding out things as we go.

    I believe my OB should've done all this when I first started Clomid I had just turned 39 and had been trying for a year then. Actually, I should've gone straight to an RE. It's my fault for not knowing what might and might not be problematic. Had I known that FSH and AMH should've been tested I would have asked for it.

  • Please don't blame yourself...we should be able to rely on our doctors to tell us when issues are beyond their scope and refer us to specialists. When I was debating whether or not to do another retrieval, my RE recommended taking CoQ10 for egg quality. She suggested taking 300 mg twice a day. You may want to look into that.
