Pregnant after 35

Quad Screen Results

So after screening negative on the NT/first Tri blood tests (1 in 680 for Down's and 1 in 10,000 for other Tri's) and having negative results on Verifi test my doctor calls me today with my Quad Screen results and I screened positive for Down's with 1/92 odds. She said not to worry and she doesn't recommend further, more invasive testing, but of course I hate that I got a "screen positive" anything. Seems so strange to have one test give such a different number than the rest. I should just ignore it and think about my other tests, right? Ugh.

Re: Quad Screen Results

  • Yes, I would focus on those good tests!   I'm sorry this is causing worry.

  • Pretend like you never got it done.  The quad screening is NOTORIOUS for false positives.  I had my NT scan done and visually everything looked good, got the Harmony test because that was the one my insurance covered and got good results back from that so I didn't even bother with the follow up blood work for the Quad screen.  I know it is difficult but all signs point to a healthy baby.  If your Dr isn't worried and you trust her then I would follow her advice and enjoy the rest of your pregnancy :)  The Verifi test as you know looks at actual fetal DNA.  I would hold on to those results before I would trust the Quad screening percentages.

  • @Geeps2 yes I have seen a lot of unnecessary worry caused by this damn Quad test. Thanks -- I'll do my best to ignore it :)

  • How old are you?  The risk goes up with age.  I had 1/125 on my NT scan and then 1/28 on my quad scan for down syndrome.  The baby doesn't have it.

This. The Verify test is way more accurate than the quad screen. Don't even give it any more thought, it isn't worth the worry and confusion!

Me:41, DH:41 Positive for MTHFR mutations- one copy C677T, one copy A1298C. One daughter born on Thanksgiving in 2013. Six losses.
  • @djtippietoes I am 41 -- did you verify no downs with Verifi or similar test or did your doc advise amnio/CVS? Very glad baby is fine! These numbers for us AMA sure start out at a frightening level.

@Guennie -- Thanks! I thought about this more last night and this morning and realized there really is nothing to worry about. Still glad I got the test to rule out the spina bifida/neural tube problems (if those numbers can be trusted!)

  • They gave me the choice of a blood test (I assume Verifi for similar) or an amnio or to do nothing and just wait until he was born.  I went for the amnio because it had a shorter wait period, honestly. Did you have a detailed ultrasound?  Everything look okay on that?  I'm assuming if your Verifi test came back negative, you are fine.
  • Detailed ultrasound happens on Nov 15 along with another meeting with the genetics counselor. I'm feeling confident now though that things are okay after my initial mini freak out. :)

  • I also had the Quad Screen and the genetics counselor told me that if you are 35 you are initially always going to "screen positive" based on how they calculate it.  So every 35 year old woman goes in at a 1:244 chance.  To screen negative, you have to be 1:296 (something like that, my numbers may be slightly off).  Then they look at your blood draw and adjust the numbers, but she said very few people get out of being "positive" on blood work alone.  Then they look at your a/s ultrasound for soft markers.  So I am 35.  With my blood work, I went from 1:244 to 1:266.  Still positive.  My a/s looked good and there were no soft markers, so my ratio went to 1:532.  So no longer positive and they did not feel like additional testing was necessary unless I had to know with 100% certainty.  This is just a screener, so it doesn't mean your baby definitely does or does not have Down Syndrome.  I really think they need to let women 35 or older know about this before they do the testing.  Bout gave me a heart attack!

  • @Naniebeans Yes, the starting numbers are really horrid, and I am 41 so you can imagine my starting place. Prior to the NT scan and bloodwork my starting palce for Downs was 1/85 just based on my age. After both tests it went to 1/680 = screen negative. But this Quad test seems to give bad ratios quite frequently.
