Cerclage advice

I'm 13 weeks pregnant and I had my NT scan today. My cervix is already 2.8 cm. it was 4.3 @ 20 weeks with my son. They did the scan abdominallu my so I'm going back next Wednesday to have a vaginal u/s and to remeasure. The dr wants me to consider a cerclage based on the results.

I had a short cervix with my son but we didn't know until I was 29 weeks. I was on BR from 29-37 weeks with him and my cervix stayed around 1.4. I ended up being induced. I never went into PTL with him just had a shorter cervix. But it's already noticeably shorter than where I was with him.

Has anyone had a cerclage placed this early? Were you on BR the rest of your pregnancy? Was it painful?

I'm hoping the measurement was just off but I'm doubtful. Thanks for any advice!

  • I got mine put in at 14 wks as a preventive because I had a 21wk loss last pregnancy. It actually more success full doing it earlier then late anything done past 18wks is considered a rescue cerclage and success rate goes down from 85-90% success to 60-40% success. Pre-cerclage my cervix was 4.5cm at 21wks I started to show signs of shortening so was out on bedrest from 21-34wks. I am currently 35wks and get it removed next wk. The cerclage procedure it's self isn't painful and is very quick.

    Laura you will be ok. Just take it easy till you get anwers whether they going to put it in soon.

    Make sure you both ask to be put on some form of progesterone. I currently take 17p injections every wk. I started at 16wks my last injection is also me t wk. cerclage and progesterone are usually offered together for better results.

  • My dr said because of my history the progesterone wouldn't be helpful. I can't exactly remember what she said.

    She does think me having a history of 2 FT pregnancies is a good thing but it's obviously much shorter than it was with my son. Today the u/s was done abdominally which I hear isn't the most accurate. I'm hoping it will be longer when done vaginally.

  • Yes ab u/s are not accurate and it also shows the cervix longer :-( even if it shows shorter vaginally sounds like your dr is ready to be active and give you the cerclage. Please keep me posted
  • Oh I remember. Progesterone should keep you from contracting but I never did (I didn't have PTL) so that wasn't what shortened my cervix.

    Really about the abd vs vag u/s? I'm on a spin off FB board from TB loss boards and a girl had her cervix measuring 2.5 abdominally and 3.5 vaginally due to the way her uterus tilts. I'm hoping maybe the same for me but I'm not holding my breath.

  • Yes, thats how they missed my short cervix with my son that i loss. The abdomen sono showed a 3.8cm cervix. 4 days later i was dilated 4cm...  Progesterone is also meant to strengthen your uterus to help prevent contractions. Woman with weak or short cervix often get silent contractions. Progesterone also can sometimes give it length. 
  • I had two cerclages placed with both my pregnancies and was on the P17 shots. Progesterone does help, request that from your doctor. The P17 shots or progesterone is backed by March of Dimes as the only medicine proven to increase the succeeds of PTL ( preterm labor) get the cerclage too. Easy, painless and you won't be on bedrest unless your cervix continues to shorten. Which may be around 28-32 weeks.

    The odd thing is I never had PTL w/ my son. I was in the hospital for 6 days on the monitors with no ctx. (This was from 29-36 weeks of at home and hospital BR). My cervix went from 2.6 to 1.5 and stayed. They put me on procardia too. Once off BR I never went into labor (it was lifted at 36w4d) and I was induced at 39w2d although I was 5cm dilated at the time of induction. We don't know if this is just how my body is (short but strong cervix). Unfortunately, it's not something you really want to take a chance with.

  • Definitely go for it if your dr suggested it. I had a preventative placed at 13 weeks from a loss at 18 weeks. Had no complications or pain after the procedure. No bed rest or p17 or any other supplemental meds. Removed cerclage at 37.5 weeks and am enjoying staring at my rainbow newborn. ;)
  • Thanks Jen! I go tomorrow for her to recheck. I've had a lot of cervix twinges and just soreness in my groin/pelvic area. You would think since this is pregnancy #3 I would know what is "normal" and I know these things are normal but I never had them this early. I've decided I will do the cerclage if she thinks that is what I should do. I'm super nervous about tomorrow's re-measurement.

    Glad to hear it wasn't painful because from what I've read online it sounds definitely doesn't sound pleasant.
