PIO question from IF board

Hi Ladies! Hopefully I will be officially joining you all in a week or so... but in the mean time I have question on the PIO.

When did you stop PIO? Did you transition to crinone from PIO? Anyone have a reaction to PIO in Sesame Oil?

I had my first beta yesterday and go back Monday for the second. I don't mind the PIO shots at all, but yesterday started getting giant itchy welts at all the injection sites. Doc is pretty sure it's allergy to Sesame and is looking to see if they can find me PIO in a different oil but apparently there's a national shortage...fortunately benadryl seems to be working on reducing the inflammation and itchy. But they would prefer I switch to crinone just to be safe. The whole reason for going to PIO this cycle was because they didnt think I metabolized crinone and I had major irritation and break through bleeding - even on 2 crinone a day.

My inclination is to just man up and deal with itchy-butt syndrome so long as no other reaction is occurring.

I was on crinone till about 10w, pio stopped at 12w2d, and I'm still on 400mg oral prometrium. I didn't get welts from pio but around 11 weeks I broke out really bad all along my right side. Can they switch you to pio in ethyl oleate?

I was on PIO in ethyl oleate until 10 weeks. I started getting welts and bruises after a few weeks but they weren't itchy. I got the PIO from Rosemont Pharmacy and I know they make it in their pharmacy on site. I live local but I am sure they ship- let me know if you want their info. I think it was around $50 a vial.
I also bled through the Crinone so I wasn't prescribed it again.

They are trying to find the ethyl oleate, but apparently there is a national shortage. The poor IVF coordinator in our clinic is tasked with locating. I honestly dont care what the price is. This is our first hint of success and their is reason to believe the PIO has played a part in it...

I had the same reaction to the sesame oil, and yeah...national shortage of anything else. So I powered through and took some benadryl when it was really bad. I keep on PIO until about 7.5 weeks and have about 1 more week of my progesterone/E2V suppositories. Hope it gets better for you!

I have contacted several compounding pharmacies in our area (fortunately we have a ton of options) and will see what they say. I'm prepared to power through them so long as the reaction doesnt get worse. This will be the second "oil" form allergy I have. A few years ago I discovered I'm highly allergic to Avocado oil. You would not believe how many shampoos, conditioners, and hand lotions contain avocado - not to mention the current health craze with Avocado - it's in EVERYTHING. Hopefully Sesame is a little less popular and easier to avoid.

My nurse said I'll stay on it until 10-12 weeks along. My understanding is that allergic reactions to the sesame oil are fairly common and switching the oil base can help. I think ethyl oleate is least likely to cause a reaction (and is also a thinner material) but other options are cottonseed oil and (I think) linseed. I would def call your RE's office if you are having a reaction though.

We found ethyl oleate, olive oil, and cottonseed oil.

It seems its just the larger corporate pharmacies that dont have it. Walgreens is only making the sesame one right now. CVS said they could get the EO, but not until mid next week. Mandel's in NJ has three varieties - but they are expensive in comparison - $55/bottle compared to Dobbs at $35.

Honestly I dont care the cost. I just want to not have an itchy back side anymore!!

Do you have an Avella pharmacy (specialty pharmacy) in your area. That's where I get my EO and I have the least issues with it. I have reacted to 3 other types of oils and so far this one has been great. No welts just sore from the injection and no rashes!

CVS couldnt get it to me before mid next week though.

We're going with the ethyl oleate to minimize the potential for cross reactivity .

@bhead81 I'm in Ohio, there is an Avella in Columbus about 2 hours away. I'll have to call and see if they stock it. They are more expensive though. Dobbs is $35 for 10ml vial.

I am about an hour from mandells in nj and have nothing but excellent things to say about then and their customer service- even though they are more expensive!

I was on Endometrin twice a day and PIO at night starting the week of transfer. I was taken off PIO after our second heartbeat at 7w2d. I was on Endometrin only then twice a day until I ran out last week (9weeks) and switched to Crinone once a day. I'll be on that until Oct.22 approx 11 weeks then I'm done yay!!!

I occasionally had itchy and welts from PiO sometimes we would use one side two nights in a row to give some rest

First of all, congrats on your beta!

I, too, had a reaction to sesame and switched to olive oil with much better results. We found it through Walgreens Specialty and had to pay OOP since it was out of network, but it was worth it!

I switched from PIO to crinone at 10 weeks and did crinone until 12 weeks, at which point I stopped all meds.

I am on Progesterone in Ethyl Oleage for just Two more days!!! It has been 10w6d of Shots. My nurse highly recommended ethyl oleate...thinner and easier to inject. GL!
