Pregnant after 35

Lovenox advice?

Hey ladies, Hoping someone might be able to help.  I have a really complicated medical history, but after 5 miscarriages and being 42, I'm pretty much at my last shot here.  Even though my RE thinks it's questionable if it's 'needed' she has agreed to move forward with my plan to introduce Prednisone + Lovenox into my next IVF cycle.  Prednisone is not the issue (I have it already) but she wanted the hematologist to weigh in on dosing, when to introduce, etc.  The hematologist I saw back in Dec does not return calls.  I went back to my nurse practictioner and she wasn't any help. I keep going in circles and the clock is ticking.  Any day I am going to get my AF and the cycle train begins and I don't know where to turn.  If you are on Lovenox, who prescribed it and how did it come to be?  Desperate.....

11/11 Hysteroscopy revealed uterine septum, which was repaired
12/11 1st round of Clomid (100mg)=BFN
1/12 2nd round of Clomid (100mg)=BFN 
2/12 Repeat HSG=All clear 
2/12 IUI with injectables #1=BFN 
3/12 natural cycle=BFN 
4/12 IUI with injectables #2=C/P 
7/12 IVF #1=30ER, 9F, 3T, 1 Frostie 7/29=BFP! 
8/27/12 No Heartbeat. Waiting to M/C 
9/13/12 D+C
12/12 Operative Hysteroscopy 
1/31/12  IVF#2 ...BFP 2/6 m/c 2/12 methotrexate due to presumed ectopic
July '13 IVF #3: 13R, 10M, 8F=BFP
1st Beta=549  2nd Beta 594 3rd Beta 422. M/C #5
IVF#4 in November=BFP…TWINS!!!!!

    Hi - I am not pg currently but saw your subject line pop up so I am chiming in.  I took Lovenox throughout both pregnancies because I have a history of DVT.  I knew it was a possibility from the time I had my DVT, which was several years prior to my first pregnancy, so my situation is of course different.  My OB prescribed it, although she did consult with a hematologist about it as well.  I took it from the first official OB appt (around 8 weeks each time) until the final few weeks of each pregnancy when I was switched to heparin.  So, I did not take it from day 1 of AF or even the first several weeks of pregnancy, and all was fine.  Not that this means that is the case for everyone, of course, because your reasons for taking it are different from mine.

    In any event, your RE should be able to reach out to the hematologist and get a quick answer.  Doctors will answer other doctors much more quickly than they will answer us, which is annoying but so it goes.  I would push the RE to make the phone call because it could probably be resolved really quickly that way.

    ETA:  My use of Lovenox was "questionable" too, because I had a DVT but tested negative for FVL and all the other known clotting disorders.  The hematologist still felt it was better to take it than not.  Hope that helps, and best of luck to you!

  • Hi. I am on lovenox now. I am 37 and pregnant with my 6 th. I have factor 5 which makes me more prone to blood clots. In fact, I have one now. I was prescribed my dosage through my hemotoligst/oncologist. And, no, I don't have cancer, my doc does is specialized in both. I also see my regular ob, and a perinatologist . My other docs leave the lovenox dosages up to my hemotologist except my perinatologist is having my hemo. Switch me over to heparin at 36 weeks. I have to give myself two shots daily. I would switch hemotologist a if you could. He should be calling you or your other docs. Mine calls me directly to check up on me about once per week. Since you too tested positive for factor v, you should begin lovenox prior to pregnancy, because you are also over 35 which is a risk factor in itself. I had five miscarriages all due to factor v and not starting lovenox prior to getting pregnant. I would switch doctors, that is my opinion. If you are not being taken care of you should find doctors who will.
  • Oh, and my docs said that if I wanted to get pregnant again...I don't....I have to start prior to pregnancy.
  • I have used Lovenox in two of my successful pregnancies and will likely use it again this time (6 weeks, 1 day pregnant today).  I use 40mg once a day starting from when we see the formation of pregnancy on ultrasound (a normal looking gestational sac).  I use the Lovenox all the way until about a week before my scheduled inductions and then am supposed to go on heparin.  the half life of heparin is smaller than Lovenox so this makes an epidural possible.  
    I also take baby aspirin while TTC.  Baby aspirin has been the one common denominator in all my successful pregnancies.  Without it, I miscarry.  Ask your RE if taking baby aspirin might be a good move.  In my case, it doesn't hurt so that's why we tried it.
