mono di question

hi all, i'm 22 weeks with mono/di boys and have been going for scans every 2 weeks starting at 16 weeks.  today was the first day they told me that they are only checking fluid this time and will check growth at next scan.  i was so disappointed!  they said that for mono/di's they check fluid every 2 weeks and growth every 4.  is this consistent with your care?  

also, i've been reading about the MCA/PSV doppler?  that they should be checking that each time as well?  they do check cord flow and watch it going around the bladder, and i'm not sure if that's what they are looking at?  i know they each have a 3 vessel cord.

  • Unfortunately I have to info to offer. I'm only 14 weeks with my mono/di twins. But I wanted to tell you good luck and I hope you and your docs figure out what's best for you and your babies. I'm going to check my future appointments to see the details of them because my doc tells me what's going to happen at each appointment. I'll let you know what my MFM has planned.

  • Hi there! I am 24 weeks with mono/di boys as well and instead of every two weeks I go in every three weeks and they do about a 30 min scan and he checks size and fluid. He also check blood flow every time. They told me that unless they notice signs of TTTS every three weeks was fine up until 32 weeks and then I will start going in once a week! Our sweet boys are doing really well and as of 23 weeks 5 days they were measuring in at 1lb 5oz each! Hopefully that helps a bit...so excited for you and your little men! Wishing you the best with your pregnancy and those sweet boys!

    thanks! at my consult in 2 weeks after the growth scan i'll ask her what to expect the rest of the pregnancy.  i've been so concentrated on getting to 24 weeks that i haven't thought much past that.  your boys sound like they are doing well- congrats!

  • At 22 weeks I started going weekly for fluid checks because of TTTS.  I'm not sure what the plan was if I had not had issues since that was only the 2nd time I had seen my MFM (it was 4 weeks between scans up until that point).  I think growth scans every 3-4 weeks is within normal.  I had them every 2 weeks since there was a growth discordance. 

    The umbilical artery dopplers should be done with every fluid check because it basically tells you how the blood flow is directly to baby.  I don't believe the MCA is checked routinely as it can be an indicator of fetal anemia if the baby is shunting blood to the brain.  My donor twin had it checked a few times because he had absent and reversed umbilical doppler readings.

  • I am also having modi boys and I'm 22 weeks. I am on the same type of schedule as you. They told me that's what was typical unless problems arise.
  • I am having di/di b/g twins and I know that I am not as high risk based on that but do have cervix issues that have caused me to have more u/s. My growth scans are every 3-4 weeks and now I am on weekly u/s for fluid checks. I am 31w3d.
  • I am carrying momo girls and I go every 2 wks but he measures them every 4. He does do the doppler on the cords and looks at fluid every time.

  • I am having mo/di boys and I went for a full anatomy + fluid scan at 16 w. this week 18 is just fluid and next week will be anatomy + fluid. So we alternate, I get AFI checks every 2 and anatomy plus AFI every 4. I think the AFI appt is including a cord Doppler too

  • Sounds appropriate.  I'm 30 weeks today with Mo/di girls.  We had scans irregular scans up until 12 weeks to make sure there was a membrane between them.  Then had a scan at 16 weeks, anatomy scans at 18 and 20 with a perinatologist.  From there the plan was scans every 2 weeks to check for TTTS.  I scheduled my 22 week scan a bit early (thankfully) and at that point  stage II TTTS had developed.  We had surgery the next day to treat it.  From 22 weeks - 28 weeks, we had scans every week to check fluid/bladders/stomachs.  Every other was a growth scan as well.  At 28 weeks, we we're all doing so well we've now been switched back to scans every 2 weeks!  I know it seems like a lot of Dr appts, and scans, but I'm proof that close monitoring can catch a problem early enough to successfully treat a problem!
  • You can't do growth scans too close together (NMT every 2 weeks), the numbers can get skewed and cause worries for issues that don't exist because the scans were done too closely together, so full on growth scans every 4 weeks for now is probably just fine.  The fluid checks and bloodflow checks are typical and will also alert to any issues.  Those smaller checks increase when you add routine NSTs to the mix.  It's usually called a BPP or biophysical profile.

    TTC since May 2006. After 3 failed Clomid cycles, 2 failed Injectibles/IUIs, 2 failed IVFs and 1 failed FET, we moved on to adoption!

  • The MCA/psv checks for TAPS (twin anemia polycythemia sequence)(a different form of ttts) which is when the red blood cells on one twin go to the other twin. The only way to detect it is through that....and it is a different Doppler than the umbilical one. This one looks at the blood flow.in the brain. I would definitely be pushing for it. It is thought that the acute form.of ttts at birth that supposedly a surprise is actually TAPS in most cases. As TAPS is detectible I would absolutely want to know about it. TAPS was not a problem.with us, but I still wish I had pushed harder for my practicions to do the MCA doppler. I was told it was not necessary. However I am part of a modi fb group and we have been seeing a lot of TAPS cases that were not seen until.birth....it seems like TAPS is not really understood and/or acknowledged by a LOT of practitioners a d therefore swept under the table as unnecessary. The fb group I mentioned has lots of different medical papers that talk about various things related to modi pregnancy.

  • That's what my OB did until they showed possible early signs of TTTS then we went to weekly fluid checks.

  • This sounds very similar to my schedule. We ended up monitoring cord flow a bit more often because baby B lagged behind in growth, but there was no issue.
