Grandparent survey

I would like to make a survey of sorts to give to DD's grandparents for her baby book (that I haven't started on). Please help me come up with questions.

This came to mind because I don't have any grandparents left, and truth be told, I don't know much about the ones I had. I know my dad's dad's favorite song was Daydream Believer by the Monkees, but that's all I got. I want her to know their favorite songs and colors and places. What else could I ask? TIA!

Have them write a short narrative about the day your daughter was born, what they like to do with her, etc. so that way she can have memories with them.
