jumping the gun?

Any donor sperm couples go that route BEFORE having a TESE or seeing if TESE was possible? Or are you all DS was the absolute last resort to try for the take home baby?

We're prepared to use donor sperm, even have a vial on "courtesy hold" and I've been ok with DH not wanting to go through anymore testing. BUT...what if? What if they could find some that were actually motile (because that's our main issue, not our only issue, but the main one).

We're 100% oop and not that it should come down to money, but, I don't know what to do now.

                      Do we go IUI with donor sperm for significantly less cost 

                                                or

                     Attempt TESE and go IVF

Oh my...I'm making myself crazy!

  • We had 4 IVF cycles and a natural bfp (early mc) before we tried DS.

    My H had the sperm DNA frag test done which revealed really bad results. His frag was the 2nd highest his urologist had ever seen. There has never been a documented full term pregnancy with anything even close to this level of frag. That was enough for my H. His urologist said she would try a TESE, but admitted she didn't think we would have significantly better results.

    Would this test be an option for your H? Maybe it would help with your decision?

    Me (32) DOR, elevated NK Cells/ 2 copies of MTHFR mutation/ MH (35) azoo/high DFI   (TTC#1 since 2009)

    IVFs#1-4: (4/10-2/12) all BFN

    Surprise Bfp (9/11) - c/p

    DS IUI#1-2 (9/12, 10/12) - BFN

    DS IVF: (11/12) - BFN

    DE/DS IVF#1: (10/13) - 2 day 3 embies transferred-BFFN

    FET of 2 day 6 blasts: (12/13) - c/p

    DE/DS IVF #2: (4/14) - 1 day 5 blast transferred...BFFN...again.

    FET 5/14: 1 day 5 hatching blast transferred...another BFFN

    Repeat SHG 6/14-normal / Endometrial Receptivity Array biopsy 7/14-Receptive Uterus

    New RE, additional testing reveals elevated NK Cells

    FET of 1 day 5 blast (RE recommends transferring 1 due to elevated NK cells) with lovenox, steroids & intralipids in October

  • Hello and I'm so sorry you guys are going through this. IF in general is cruel and sucks and MFI in particular really sucks..just because there are so many unanswered questions and so much more they need to do in terms of treatment and diagnosing. All my info is in my siggy but basically we had a big fat failure with our IVF. DH had had multiple surgeries and we were just physically and emotionally drained following the FET BFN. Our doctors couldn't give us the reassurance we needed that our second go at IVF would be any better. It was super tough because DH does produce SOME motile sperm but inconsistently. We were just at a place where we wanted a baby and we thought if we go through IVF again, and get another negative, we didn't know if we could survive it. The first one was crippling enough. It's obviously an individual choice but we went with the DS IUI and I am currently 26 weeks pregnant from our second IUI. GL to you with whatever you both decide! And hugs for having to even make this decision.
    TTC since June 2011
    DX: DH (30) severe MFI, severely low count & low motility
    Me (32): all clear
    Appt with Urologist 5/21/12: exam, ultrasound, bloodwork all normal.
    Testicular Biopsy with TESE on 6/8/12. good sperm found! (left side only) froze sperm, failed thaw test :(
    Orientation for IVF/ICSI on 6/13/12. Waiting for the green light following biopsy results...results show adequate sperm production both sides.
    2nd SA 6/18/12: sample is "adequate for ICSI"
    Plan: IVF/ICSI July 2012!
    ER: 7/26/12. 15 eggs retrieved, all mature.
    TESE/TESA/aspiration from epididymis, no motile sperm found :(
    froze all eggs, the saga of praying for good sperm continues.
    8/3/12: 2nd opinion from MFI uro on biopsy slides. Suspects "partial late maturation arrest."
    Plan: more SAs, third biopsy/TESE with frozen back-up either from DH or DS.
    SA 8/17/12: Zero sperm
    SA 8/23/12: Zero sperm
    9/26/12: SPERM FOUND! 15 eggs thawed, 12 survived and were ICSI'd, only 3 fertilized normally. Refrozen as embies and will thaw in Nov. Please survive and grow!
    All 3 survived the thaw on 11/15/12!
    FET 11/17/12: transferred 2, one 4B, one 4C. Beta 11/30:BFFN
    moving on to DS
    DIUI#1 2/18/13,50mg Clomid cd 3-7, Ovidrel trigger, Beta 3/4/13: BFN.
    DIUI#2 3/19/13, 50 mg Clomid cd 3-7, Ovidrel trigger, Beta #1 (14dpiui) 4/2/13: BFP!!!! 150. Beta #2 4/4/13: 420 Beta #3 4/8/13: 2691. Beta #4 4/15/13: 15,086
    1st u/s 4/8/13 shows one gestational sac
    2nd u/s 4/15/13 shows yolk sac, fetal pole and early heartbeat
    3rd u/s 4/25/13: measuring right on track. Heart rate 148 bpm
    A/S 7/22/13: IT'S A BOY!!

    @melkel559  This is what we're trying to avoid. IVF's that are unsuccessful to then end up with DS and IUI in the end anyway...I feel like this is our path...thank you for sharing your story. The end result we're after is a take home baby to raise together...at least with DS, it will be 50% our genes and we won't have to worry about a mom deciding at the last minute to keep it.

  • I'm so sorry you and your DH have to go through this!  My hubby had the same issue.  We went through with the TESE and found out there was no sperm so we had to go forward with DS anyway.  Sounds like your situation is a little different but still kind of similar.  My DH was a real trooper with the procedure but was out of work for a week and he seemed to be in a lot of pain.  It was something that my hubby felt he needed to do so we know.

    It's all about what you feel would make your family perfect for you.  Good luck with your journey!
  • My DH had two 0 sperm count SAs before we went TESE.  We wanted to exhaust all of our options for a full bio baby.  There was no blockage, so ultimately the 0 sperm count was because DH does not produce sperm (which we found out after the TESE).  It's certainly a personal decision.  DH and I were willing to try everything first, and it was all covered by insurance.  Had it not been covered, we might have thought differently.  We've made the decision to move forward with donor sperm and are confident in our decision.

  • @meck716 Thanks for sharing your story. We did decide to move ahead with DS. In fact, I'm using OPK right now waiting on the surge!! Donor vial is at the lab waiting on me!  Very excited!!

  • We opted not to do the TESE because of all the other tests done. The doctor didn't think we would find any since he could rule out blockage. Our insurance probably would not have covered it either. We've done 2 IUI with donor and may do a few more but that's it, IVF is not in the cards for us.
