Pregnant after 35

Over 35 always means Level II ultrasound??

My OB changed my u/s from a regular one to a level II she said it was routine because I am over 35.She said my NT scan and bloodwork results were great.  The NP was the one who had me schedule my other one.  So does everyone over 35 get a level II u/s?

  • Most of the time they only schedule a level 2 if you screened positive for a risk. I'm not sure why your dr would say it's routine. Must be routine for that particular practice but it certainly isn't routine everywhere.
    Me:41, DH:41 Positive for MTHFR mutations- one copy C677T, one copy A1298C. One daughter born on Thanksgiving in 2013. Six losses.
  • It seems to routine at my hospital. They ordered a level II for my A/S, and this was before I'd even gotten the results of my NT scan.
  • In our area, it's pretty standard to do the Level 2 regardless of age.

  • It's pretty standard at my office, they don't even bother with the NT scan or blood work. They do it between 15 and 20 weeks. I get mine I'll be about 18 weeks and won't get a growth scan as the Peri will do it all at the same time.

  • That's not a stupid question :-)  I didn't know there were different levels until a few weeks ago :-)

    My understanding is a level 2 US looks for more soft markers for DS and other chromosomal abnormalities that a level 1 US.  They did schedule a level 2 for me only because one of my blood tests came back with unusually high hormone levels (HCG and inhibon).  The genetic counselor and my OB were very clear that the US looks for markers but is not a substitute for an amnio should you want definite answers about chromosomal abnormalities that an amnio detects.  

    At my hospital, a perinatologist looks at the images immediately during a level 2 US while they are often looked at on a later date for a level 1 US.  

    Again, I was only offered one because of a blood test result.  However, level 2s don't take any longer than level1s (what I had with my first pregnancy) and you do get to speak with a doctor immediately should you have any concerns. :-)

  • This is my second pregnancy and I am 39.  This time because of complications with my first I am having tons of testing, but the level 2 seems to be standard with most of the obs I have known.  All of my friends who were 32 or older have had one, even when the down's us was negative or low.  They now do the harmony blood test which is 90% accurate for downs, 80% for one of the trisomy and a little less for the other trisomy.  It is not a bad thing, but usually they recommend it for women of an advanced maternal age, just for an additional marker because our risks are greater for pre eclampsia, gestational diabetes etc.  I look at it as an additional reassurance all is well in the womb.

  • I'm 36 at 29wks and never had one.
  • I had my first LO at 38 and never had a level 2. I'm pregnant with our second at 40 and my OB hasn't mentioned one. I did accept the NIP blood test for chromosomal problems this time.

  • It seems to be the standard of care here locally for any age to have a level 2 scan between 16-20 weeks.  They are looking not just at Down's markers but also for heart defects, long bone defects (club foot, etc), and other abnormalities.

  • My doctor's office automatically scheduled me one and I was just 34, but would be 35 when I gave birth, they said it was normal. I loss that baby, however I am pregnant again and they automatically scheduled one this time too. They said it was normal routine for over 35.
  • Congenital heart defect. He had transposition  of great arteries and open heart surgery at 5 days old :(.. ..but everything went great :)

  • My OB does Level II ultrasounds for all of her patients, no matter the age.
  • @aga60154, I am approaching 29 weeks pregnant and am having a baby with a CHD. Did you find out while you were pregnant that your baby was going to have a CHD?
