Intro - 2nd pregnancy after CI

Hi everyone!  I am a long-time lurker and posted a little bit when my DD was born a few years ago, but my screenname had some identifying info so I made a new one (and I love Friends!).  DD arrived at 23w4d due to cervical incompetence.  I got an emergency cerclage at 21 weeks which held for a couple weeks until she arrived.  We had a 4+ month NICU stay but she is doing great now and almost 3!  

I recently found out I am expecting #2, and was hoping to get some insight from moms who've had pregnancies after going through cervical incompetence.  I saw my doctor last week, and the plan for now is to go in every two weeks for check-ups, and start weekly progesterone shots at 15w.  Unless I start showing signs of CI again, we likely won't do a cerclage this time.  I did some Dr. Googling and it sounds like most people had a cerclage for subsequent pregnancies after cervical incompetence, so I wanted to hear others' experiences.  Thanks in advance!

  • Welcome! Another Friends lover here :). Please stick around and keep us updated! I had DD2 at 26w3d and they can't find a cause so are calling it IC. It was a shock since I had no cervical issues with DD1 though she was born still at 31 weeks. As we consider another pregnancy, needless to say we are terrified. From what I have seen/heard/read, it seems like it's just a crap-shoot and no one knows what the right combination of progesterone/cerclage/bedrest will be. Ugh.

  • Love your siggy name! I had my DS at 26 weeks after I had PROM at 23w5d. With my 2nd pregnancy, my cervix started to shorten and had funneling at 20 weeks and I was placed on SBR all the way to 37 weeks. By the time I hit 28 weeks, my cervix was barely there but closed the whole time. I actually made it to 39w4d! I also had P17 shots for 20 weeks starting at 16 weeks.

    MFM did not think I needed a cerclage when I got pregnant with baby #2 and when I asked her if I had IC, she said that I had deficiencies, but would not classify it as that. She said if we were planning on baby #3, she would definitely do a preventative cerclage. Well, I will never have one because we are definitely done having kids. I do think that it is common practice to have a cerclage for subsequent pregnancies especially if that was the cause of prematurity. My MFM more aggressive in actions though especially if there is history of premature birth. She was with me which I was totally fine with-and I feel like I owe everything to her for having a FT baby.

  • congrats on your pregnancy!  So great to hear that youe preemie is doing so well too! 

    I had a classic case of IC.  I had painless funneling and dilation starting at 21 weeks, including a dramatic shortening of the cervix at 22 weeks where I went from 2.4cm to .4cm in 5 days.  I have had several MFM consultations in preparation for another pregnancy and have had the whole range of recommendations.   For many reasons I decided to be treated very aggressively and had a transabdominal cerclage (TAC) placed.  

    Its seems pretty rare that true IC cases are isolated to one pregnancy without affecting the subsequent pregnancies.  The trick is understanding whether your premature delivery was the result of classic IC.  I recommend hashing this out with several OBs and /or MFMs to really dissect what happened and why it happened.
