Pregnant after IF

Successful IVF protocol?

I hope you don't mind the intrusion, but I was hoping some here might be able to lend some advice to a friend of mine. She has had 2 IVFs and 2 FETs. The IVFs resulted in early miscarriages, the FETs were unsuccessful. She has an apt with her RE next week, and is wanting to discuss where to go from here. I thought maybe if some of you shared the protocols that led to success for you, she might be able to discuss these alternatives with her doctor. So, if you don't mind, I'd love to know what your IVF regimen was that led to your pregnancies. Thank you so much, and all the best to you!

  • It really depends on her diagnosis, whether or not there is a male factor, her age, and what was previously unsuccessful for her. For what it's worth, I'm early DOR, was under 30 at time of transfer, in good health, and no male factor. Antagonist protocol worked beautifully for me. I also did acupuncture. Maybe she can add that?

  • I second pp that it depends on diagnosis. I am DOR did a microdose lupron flare that failed and had success with antagonist. I also did acupuncture for several months leading up to the Ivf and am continuing through my pregnancy.
    Me (37) DH (39); PCOS changed to Unexplained, changed to DOR in 2012 (finally a correct diagnosis!); 
    Started TTC 2009 with RE after 6 months.  
    Clomid + Trigger x2; 
    IUI + Femara x1,
    IUI + Follistim x2;
    IVF #1 (MDL) February 2013- BFN.
    IVF #2 (antagonist) May 2013, First BFP of my life. 
    Identical twin miracle BOYS (!!) headed our way- due date is technically 2/4/14 but c section is scheduled for 1/7!

  • Agreed - a successful protocol is going to depend on her diagnosis. She may also want to think about getting a second opinion from another RE at a different clinic if possible.

    FWIW, our diagnoses was "unexplained IF" and long lupron worked for us.

  • We did long lupron w our successful cycle. Im also working with a reproductive immunologist and I feel that contributed to our success.

    Feb'12- July'12-testing(all clear minus slight arcuate ute), 3 IUI with clomid all BFN

    ET 9/14 2 embies transferred. 1 10cell Grade 4, 1 8cell Grade 4. No frosties. BFN
    IVF#2 Antagonist protocol plus baby aspirin- ER 12/5-16R, 12M, 8F!

    A second opinion with another RE is probably a good idea.  If I were your friend, I'd probably seek further testing for recurrent pregnancy loss (if she hasn't done that already).  Also, if she responded well to her first two IVF cycles, a change in protocol might not be helpful (she obviously had frozen embryos, which is great).  If she is getting a good number of embryos to a 5-day blast stage, CGH or PGS to screen for chromosomal abnormalities might also be helpful, as chromosomal issues are a common cause of early miscarriage. 

    However, to answer your original question, a long lupron protocol is pretty standard for your friend's situation.

    3/27 ET: transferred 1 embryo, beta 9dp5dt=163, 12dp5dt=639
    4/25 1st ultrasound at 7 weeks = identical twins with heartbeats?!!!
    PPROM at 31w, delivery at 32 weeks of two beautiful girls

  • I had to go to three different doctors before one figured out what would work for us.  So I definitely encourage your friend get another opinion on her case.

  • I definitely agree on an RPL panel.  After my first failed IVF I had one done and found out I had 2 clotting disorders.  I think meds for those contributed to my success!

  • I went through several cycles and two losses before we found success. The things that we did differently this time were:

    estrogen priming (no BCP) in addition to Antagonist protocol

    Human Growth Horomone with stims

    Endometrial biopsy (scratch) the cycle before

    intralipid infusions

    lovenox after BFP

    I will never know which of these helped, but I was ready to pull out all the stops. I hope your friend finds success soon!
