  I'm guessing it depends on the kind you purchased. We had some that worked with a mattress only. It just had metal bars that went between the mattress and box spring to hold it in place.

  My DD only had a mattress and box spring and the bed rail fit between the two

  We have an ikea bed with wood slats and a mattress and the rails work fine, but technically are meant to go between the mattress and box spring.

  DD twin mattress is on the floor and the rail goes between the mattress and floor just fine.
