Twins - Measuring 8 days apart (insert worried mama)

Hi Ladies,

First, please ignore my siggy. I am originally a May, 2011 mama and I haven't really been on TB since my son was a little. I'm on my 2nd pregnancy now with twins. 12w1d. Yesterday I had an u/s to check heartbeats and found that Baby A and Baby B are measuring pretty differently. Originally, when we discovered I was pregnant with twins I was 5w4d both twins were implanted and we scheduled an appt for 8w. At 8w. Left baby (no a/b yet) was measuring at 8w0d and R baby was 7w6d. Doctor wasn't concerned. Now at 12w0d Baby A is measuring at 11w4d and baby B is at 12w5d. I was referred to a specialist to see if the difference is something we should worry about.

I am wondering if anyone had such a large variance this early in the game. I still don't know if I have fraternal/identical twins and I really am not 100% sure if they have seperate placentas or not. My original u/s looked like they were identical, but my doctor wouldn't say anything about it because she wasn't really sure.

TIA

Re: Twins - Measuring 8 days apart (insert worried mama)

  • Sorry you are worried. Hopefully the MFM can give you some answers and hopefully there's nothing wrong. Mine measured about 5-6 days apart around that gestation. Also wanted to add that it's really important for them to diagnose the chorionicity (# of placentas) asap. If they looked like they were in the same gestational sac (one big black hole with a thin membrane or no membrane) they are monochorionic (identical) and need special monitoring.

  • I have no advice to offer, but just wanted to wish you luck and hopefully the MFM will have some answers!

  • ok - so this is measurements from your OB's kind of janky machine? If so, I wouldn't borrow worry. Your MFM/Specialist/Perinatologist will have a much better machine to look at them on. Even at my MFM's he told me that he will always have a few days of discordance because one twin is closer to the wand than the other. At my 13ish week appointment with the MFM, he had one measuring 12w6days and the other was 13w2days - so a few days off with the super power machine. Definitely get in with your specialist asap - it sounds like your OB is kind of clueless.

It was on a fairly old machine, but would the difference still look so dramatic? Unless Baby "B" is closest to the wand and that is why he looks so big?

Here is the U/S pic. It's kind of hard to see "A" because he appears to be laying on his side. So I used my screenshot software to outline where they are. I'm sure you ladies are all pros at these by now.

I don't know yet. :( Been waiting to hear from the Perinatologist's scheduling dept all day. I called my OB who was sending in the referral to them and told them to try and get it ASAP, and if I could leave work right now and go I'm fine with it. I just want to get in there.

  • Based on my very uneducated observation, it looks like they are for sure in separate sacs (simply based on the black space between their heads, looks like there is a barrier), but whether they share a placenta, I've never been able to figure out that on u/s.

I think the measurements are actually okay as far as the gestational date of 12w 0d is concerned, if they have separate placentas.  One is only 3 days behind, whereas the other is 5 days ahead, so it looks like they are in the realm of normal.  But definitiely the specialist will be the one to say for sure.  I hope you get in soon!  I'd call the perinatolgists office myself and see if they received the referral yet.

TTC since May 2006. After 3 failed Clomid cycles, 2 failed Injectibles/IUIs, 2 failed IVFs and 1 failed FET, we moved on to adoption!

  • Mine measured a whole month apart. No reason given and in fact baby B from week 6 all the way to week 15 they told me would vanish or pass because he was SO far behind and had NO fluid. Well I am currently 22 weeks Both boys are perfectly healthy and the smaller is currently the bigger of the two. They still measure about 10 days apart sometimes 13 days sometimes just 3 days apart but for the first 15 weeks they were measuring an entire month apart. Some how he grew a lot and caught up (that did take a few weeks slowly he grew faster) They still don't have a clue why but we saw them on 4D ultrasound today and they looked great. Stay off google ( i made that mistake) take it day by day. If mine pulled threw after being that far apart yours can too! My U/S photos are crazy too look back on I am so thankful that they are both healthy and I hope yours pull threw as well :) size difference is not uncommon though. Today however they both were only 3 days apart so they are catching up to one another! Its hard not to worry but your difference isnt that bad its fairly common to be 2 weeks apart in size from what my OB and Peri told me.

Are the boys identical? That is a relief.  You must've been a wreck. You are almost done too!

Thank you to all the ladies that have replied. Still don't have an appt but im sure they will call in the AM.

  • We wont know if they are identical or fraternal until after birth when they test their DNA. As of right now they are classifying them as Di/Di, however their placentas (what they originally thought were two) are looking more like one but the two could have fused. They implanted almost right next to each other so they said theres a higher chance for them to be identical. One OB is calling them Mono/Di the other calls them Di/Di so who knows. I was extremely nervous. I cried so much those first few weeks and still do when I look at the ultrasound photos. I thought for sure we would lose him if not both but no they are both strong and healthy!

  • My specialist appt is scheduled for Friday. I was also on the fence about getting an NT scan because I didn't want to stress, but since that's already happened they are letting me decide after my ultrasound if I want to do the NT at that time and I they will do it immediatly. So, hopefully I will have some real answers and some adorable twin u/s's on Friday. :)
