The OP is refusing at cerclage at 21 weeks.  She is 4cm dilated.

Not going to lie, my first reaction was to judge.  But maybe it's not so black and white.  Idk.  What do you think?

  • I was going to say, it seems like 4cm dilated is a lot to be doing a cerclage, based on my very limited knowledge, and there was a post by an L&D nurse that said the same thing. Possibly damage to the cervix is a lot more likely if she's that dilated?

    Well, personally, I would get a cerclage at 21 weeks. I know many people who have had one and they have had children afterward. Just like any procedure they have to tell you what "can" happen, even if it is remote. She will most likely need progesterone also, and deft if she doesn't get it done. It's just not very smart to have a baby at 21 weeks...even 27 weeks.

    I am with you in judging this girl; but it is ultimately her decision, and she is taking the risk of her baby having major issues vs. possibly bleeding (which can happen in any surgery) or subsequent infertility...although, if you need a cerclage with one, you may need it again.

    ETA I totally missed the 4 cm dilated thing...here's what I found as far as that is concerned. Women who are more than 1.5 in (4 cm) dilated, who have already experienced rupture of membranes, or whose fetus has died are ineligible for cerclage.

  • yeah.  I feel like even with the risk mentioned, I would most likely get one.  But I'm not in that situation, so I guess I can't really say for sure.  She just seems very nonchalant about it all.  Idk though.  It can be hard to convey tone in writing.

    This, my neighbor is head nurse in L & D (I asked her, was curious) and at 4cm... it is NOT recommended, she is actually surprised it was suggested to her. Sounds like an awful situation, @kimbo1216, I don't think we should read into it if she sounds nonchalant about the whole thing.... Being online you can't tell... I am sure she was typing with eyes full of tears and just wanting to get the question out there, I would be in shock if I was even in that situation.
  • No chance I would go for it. I don't know how much funneling/effacing she has but i imagine quite a bit at 4cm dilated. At 23w2d I was 2cm dilated, 40% effaced with bulging membranes and it was to late for me. I've been on strict bed rest since and I am now 28w and 4d.

  • I would have been more likely to think MUD if she hadn't responded.  But I didn't look at her post count or when she joined.
