So I'm super excited to be in the position that I can finally quit, but now I'm sitting here worrying and being nervous about actually talking to my boss. Part of me just wants to never show up to work again and call it done lol.
Anyway, I was originally going to tell my boss that I would stick around until she found a replacement for me (which I assume would be somewhere around a month, if she hurried with the process). However, now with the situation being what it has turned in to, I just know that I can't continue even trying to stay at that job for any longer. I've only worked maybe 20 hours in the past 2 or maybe even three weeks anyway because I'm just too sick with migraines and/or dehydration to make it in the majority of the time.
I'm not sure how I should go about telling her the situation. I would like to be able to give a proper 2 weeks notice, but even if I do give her the 2 weeks notice I need her to be aware that I'm not really going to be doing her any good for those 2 weeks and I will probably work really minimal hours (like I have been already). I kind of just want tomorrow to be my last day period. The stress of that job is not doing good things for my health, I can tell, and I just really don't want to have to put up with trying my hardest to even be there, let alone get any work done for another 2 weeks.
So what would you do? How would you go about having that conversation in a way that I don't come off as a complete jerk of an employee? Or am I just going to look like a jerk either way?

Basically have a conversation about it and let him/her help guide you. It doesn't sound like they've been unreasonable with you this far, so I'm sure they will find a way to work trough another two weeks.
And congrats! I was beyond thrilled when I quit my job, so I know that feeling!!
