High-Risk Pregnancy

anyone Biophysical Profile Testing???

So today was my first visit with <?xml:namespace prefix = st1 ns = "urn:schemas-microsoft-com:office:smarttags" />OB after I got told I have GD with taking insulin at night 30 units starting tonight. Some Questions: <?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" />

Is 30 units a lot of insulin? I have PCOS so I am insulin resistance…just wondering if it’s a high number- they really don’t tell you in the office.

Dr. said that I should start doing kick counts but I have a front facing placenta and don’t feel movement all the time but just from time to time.

Anyone have a front facing placenta that doesn't feel as many kicks at 28 weeks? Is this something to be worried about?  I don't want the counting of the kicks to stress me out...

Biophysical Profile testing- What is it? What if the baby doesn't move when they are testing things.... what do they do when you are not far along enough to deliver? Do they put you on bed rest? Just getting nervous about it- they are not doing any testing until 32 weeks so I made an appointment at 30 weeks which I am 29 on Wednesday but just to prepare myself.

Also when I asked about a still born baby she said that it was one of the risks but didn’t really get into it. People with GD have you ever feared this before?? Is this why they are doing the biophysical testing?

Any words of encouragement will be helpful because we all know stress is not good for your blood sugar or the baby.

Can anyone help with some of the answers?

  • BPP or Biophysical Profile is a test to see how your baby is developing. For mine, they started testing them last week at 25 weeks. They looked for motor skills (ex. Finger and hand movements) and developmental markers (ex. breathing practice). It's just done as part of a regular u/s session.

  • Is 30 units a lot of insulin?  I wouldn't worry about if it's too much. Everyone's body reacts differently, and if you already have PCOS you might just need a little higher dose.

    Anyone have a front facing placenta that doesn't feel as many kicks at 28 weeks? Is this something to be worried about?  No experience with an anterior placenta, but as far as kick counts go, it will help if you do them shortly after eating. Babies tend to be more active then. Also, try laying on your side and being very still, really focusing on looking for movement. Sometimes being distracted means you feel movements less than you think.

Do they put you on bed rest?  A BPP is an ultrasound to check on the baby. They'll watch for practice breathing and assess muscle tone based on specific movements. I have never had an issue on any of my BPPs, but if there is the dr will evaluate the situation and you'll come up with a plan from there. I think they would usually repeat the test, unless it's an extreme circumstance.

Is this why they are doing the biophysical testing? Stillbirth is a potential risk of GD, but it's pretty rare and typically in uncontrolled GD patients. They will monitor you closely to catch issues early and do everything they can to avoid that situation.

Make sure you're asking questions of your drs when you don't understand something, and feel free to ask questions here.   • It will all be ok and try not to worry to much. I am 28.4 and on strict bedrest for incompetent since 21 weeks when I had a cerclage placed and now in hospital since 24 weeks for ptl. Rarely are patients put on bedrest due to GDM, bedrest is more freq in ptl, rom, iugr pre-eclampsia or fetal well being issues.  I am pregnant with twins and my little girl also has a placenta that limits how much movement I feel from her and yes at first it really worried me too because I feel my baby boy move all the time. The funny thing is, she is really the more active baby that moves more during her nst and is constantly flipping around, breech to transverse, but again I don't feel her. A bpp is just as they stated above, it is a wellbeing test that they give 2 points in each category (fetal movement, heart rate, respiration attempts and amniotic fluid around baby) a perfect score is 8/8 but many times this is not achived til 32 weeks due to absence of respirations.  In our instance, being so preterm, it would take something very non-reassuring to deliver.  They are also correct above is saying that it is very important to maintain and monitor closely you glucose levels. Im a nurse on l&d and yes I have seen still born deliveries but these are very rare and seen in patients with poor to pre-natal care or lack of concern to their glucose levels and diet. your doctor will schedule you more frequently now and will work closely with you and your gdm.

  • I have BPPs twice a week as well as NSTs. Combined they score on a scale of 10. If baby scored less than 8 of 10, I'd be sent to the hospital for extended monitoring for a few hours.

    I have GD but the reason I have extra monitoring is because of twins and IUGR.
