My hospitals both had Similac. They have the 2oz nurser bottles and nipples, so you wouldn't need your own.
The hospital where I had DD did not have any pacifiers. The hospital where I had DS gave him a hospital during his short stay in the NICU. It just depends on the hospital. I'd take your own just in case if you plan to use one.

Hospitals typically use the ready to feed formulas that come in their own bottle. I am pretty sure they also provided us with a pacifier, but we didn't use it in the hospital.
A lot of hospitals aren't offering pacifiers any more, so if you want to try one I'd bring it with just in case. The hospital where I delivered my first gave them out like candy, but the hospital where I delivered/will deliver #2 does not offer them except to NICU babies, so we bring out own.
