Placenta Previa + Hysterectomy

Hi all,

I was diagnosed a few weeks ago with GD and on Friday at my level 2 ultrasound I was told I have placenta previa, which right now is I guess "complete" since it is covering my entire cervix.  The doctor told me I would most likely need to have a hysterectomy at the time of the c-section.  Has anyone had experience with this?  I am not too upset about the hysterectomy, since this is our last one, but I am concerned about what I have read (a lot of blood etc) and the fact that I will have to deliver in a regular operating room and not in L&D.  Are the odds that I will have to deliver early?

I go back in 4 weeks for them to check on it.  I have not had any bleeding (fingers crossed!) and she only advised me to go on pelvic rest (nothing in vagina, no excessive walking...) but I know from reading the board how quickly that can change.  Also, anyone who has had this - were you still able to breastfeed?  I am really hoping this won't have an effect on that.

  • I have a complete previa, diagnosed at 18 weeks, and was put on pelvic rest. I then had my first bleed and was put on strict bed rest, I had 2 more bleeds, the last of which put me on HBR. I have been told that they would not let me go past 37 weeks, and am hoping to at least make it to 34. My doctor told me I would only need a hysterectomy if the rest of the placenta, which is anterior, has attached itself to my uterus.
     I would ask your doctor if they can already tell that your placenta has attached to your uterus, if it hasn't, or they can't tell, then you may not need one.
     As far as breastfeeding, I would love to know the answer to that as well. I am hoping that it doesn't cause any issues!
    Good luck to you! I hope you don't have any bleeding, but I have been told that it is pretty common with previas.

  • The only way you could possibly need a hysterectomy is if u had an accreta as well. Which is where the placenta is attached into the muscle of the uterus and sometimes into surrounding organs. If that's the case its a much more extensive surgery and that's why Its in the main OR, L and D ORs are smaller and its safer to be in the main in a case like this. Best to be prepared for the worse in doctors eyes and have it be super easy and non eventful and safe for u and your baby. There's a greater chance of needing blood as well during the surgery. As for as breast feeding obviously There's more recovery and more pain. If u had a hysterectomy its a more extensive surgery so u may have more pain, but there's no reason that u can't breast feed. You still would have ur ovaries and appropriate hormones. Sorry not trying to scare u but inform you. Hope this helps!

  • I'm confused why you would need a hysterectomy just because of placenta previa? Do you have other complications as well?
  • Thanks so much for the info...I do not recall her saying anything about accreta. However, she may have mentioned it and I don't remember, since she came at me with all of this information and I was caught off guard. She starts saying how great the baby looks and next thing I know we are talking about hysterectomies. I see my OB as well as return to MFM in a few weeks and I will definitely be more prepared with questions this time around. Thanks again :)

  • Yeah definitely ask about the accreta thing. I have a complete previa, but it doesn't appear to be an accreta as well, so no mention of a hysterectomy. I think, though, that if they do get in there and realize that the placenta is actually attached to my uterus, then they may have to do a hysterectomy. It's a risk that comes with having a previa, but not a given. Maybe she just mentioned it because it's a risk?
