GD and non stress tests

So I had my monthly check up yesterday, and my GD specialist recomended that I switch from glyburide to insulin. At yesterdays appointment, my OB went all "worst case senario" on me because I was having to increase my meds. It had me in tears and made me feel like a total failure. I really am trying to get my numbers under control bu itsmy fasting numbers and my breakfast numbers I have bee having a hard time with. I have tried everything and they keep going up. My Gd specialist who is much nicer, says its my hormones which have always been out of whack due to PCOS. Anyhow so I was originally told starting 34 weeks I would do non stress tests twice a week, well my OB has moved them up a few weeks. I start them at 31 weeks now. But other then the "worst casesenario" I got. I didnt get any whats or whys. I will find out more on Aug 5th at my first NST. Anyone been through this?

  • Sorry to hear about all the trouble. I'm not there, yet, but my doctor has warned me that I might need insulin at some point as my pregnancy progresses and my placenta gets bigger. At some point, it's possible that no matter what I do, I might not be able to keep my numbers under control. He said that I shouldn't feel bad about it because it's the placenta's fault, not mine. If that happens, it simply means the placenta is growing and that's actually what it's supposed to be doing, so that's good. I was grateful after this talk, so I thought it might help you get things in perspective, too. Don't blame yourself and don't see treatment as punishment. They are just making sure that everything's under control so that you'll have a healthy baby in the end.

    But my doctor never mentioned the non stress tests. Not yet, anyway, so I can't comment on that.

    If it makes you feel any better, I am mostly diet controlled with GD. I only take a tiny 1.25 mg dose of Glyburide at night and I have had tons of NSTs.

    I had them once weekly beginning around 32 weeks (or so?) then I started having them twice weekly, along with once a week u/s to check baby. 

    So far, everything has been fine. I am 38 weeks and a couple days, so I only have 3 NSTs left and 1 more u/s (assuming everything goes well).

    Additional monitoring is hard (it isn't my favorite) but it doesn't necessarily mean anything is wrong. Definitely bring your questions to your OB and GD counselor.

  • I was on insulin in both of my previous pregnancies, and did NSTs starting somewhere around 34ish weeks with both. The NST isn't a big deal - they hook you up to a monitor to track the baby's heart rate and possibly one to measure for contractions, and you push a button every time you feel the baby move. They are looking for the baby's heart rate to accelerate/decelerate appropriately with movement and rest. It takes anywhere from 20-60 minutes, depending on how cooperative the baby is being that day ;) The info gathered from the NST lets them know that the baby is still doing well and your placenta is providing what the baby needs. They may also add a biophysical profile (BPP) at some point. That is an ultrasound that checks for things like the baby practice breathing, muscle tone, placental calcification. Sometimes it includes a growth scan, as well.

  • I am in the same boat too. Mostly diet controlled but take Glyburide and have been having weekly NSTs and weekly ultrasounds since 32 weeks. My doctor just said better safe than sorry and she has all her GD clients have the weekly tests. Try not worry too much.

  • Sorry you are feeling worried, maybe try talking to your OB or nurse and they can walk you through your situation. I'm 35 weeks today and had to start insulin at about 29 weeks in the evening due to high fasting numbers. From what my doc and nutritionist tell me is there is not to much you can do about fasting numbers. I started NST at 32 weeks, twice a week. It's to monitor your LO. At my appointments they first use the ultrasound to keep track of fluids and the health of the placenta, baby breathing movements and such. Then they place the monitors one that tracks LO heart rate during rest and movements and the other monitor for contractions. I actually find it relaxing hearing his little heart beat. It's standard practice for my dr to have all GD mommies to do this regardless of if they are diet controlled or on insulin. Good luck to you, it's not much too worry about :)
  • I also started at 32 weeks. Sometimes they change when you start NSTs based on switching from Glyburide to Insulin. For whatever reason, it seems that insulin can affect the placenta more than glubyride/diet so they prefer to monitor more closely.

    Sorry your doctor made you feel bad :( Sometimes there is just nothing you can do and it is totally not your fault! Especially when it comes to those damn fasting numbers.
