Pregnant after 35

recommend 3 or 5 day transfer (over 40)

Hi ladies,

I've been largely absent from the boards the last few months.  After my ectopic (and 4th m/c) I needed a break.  Slowly creeping back in.  Had my ER on Friday and waiting to hear tomorrow am whether we will do 3dt or 5dt.

My previous 2 IVF's were 3dt and both resulted in pregnancies (though both m/c). If you did IVF, were you a 3dt or 5dt?  I think my clinic has changed their protocol recently and may push for 5dt. As of yesterday we were 13R, 10M, 8F.

What do you think? I keep reading conflicting info.

  • I had only one IVF, but I was five day. That IVF failed but then I had FET and that took.

  • Mine was 3 day.  I got the impression that unless I was young or had a high number of good looking fertilized eggs (didn't happen) that it would be 3 vs 5.
    Me: 37 DH: 37 TTC since Jan 2012. RE consult and initial testing done in Aug 2012: SA fine, AMH 1.1 & HSG clear - on way to IUI but got BFP on 9/10/2012. m/c 10/3/2012 at 6w5d. BFP next cycle which was a c/p. IUI #1: Clomid 100mg 12/6= BFN. IUI#2 (unmedicated) Jan 2013 = BFN. Hysteroscopy 1/25/13. IUI#3 Clomid 100mg 2/23 = BFN IUI# 4: BFN. IVF #1 April 2013, BFP!  EDD 1/12/14 with Boy!

  • When I did IVF at age 41-42, always did a 3dt-even though embries were 8 cells and looked good on Day 3.  I was never pushed to a 5dt because there was a high risk that there would be none to transfer 2days later.  We never had more than 3 embries in a cycle though because of DOR.  In the 3 cycle that we went to ER, 2 were BFN and we had one chemical PG.

    Our DE cycle was a 5dt that resulted in our current pregnancy.  GL!!!

     

    TTC since 10/09 Me-43 DH-44 RE and testing 10/10-11/10, Recommending IVF 1/11 New RE AMA and DOR-DH low motility IVF #1.1 cancelled 3/11 due to poor response IVF #1.2 May 2011, one perfect 8-cell embryo, 3dt-BFN, IVF #2.1 Converted to IUI d/t poor response. New RE 9/2011. IVF 2.2 completed using HGH,EPP,DHEA, Q-10 and accupuncture. Transferred one 8-cell, grade one embryo on 10/19. BFP 10/31/11 Chemical pregancy on 11/2/11. Started stims for IVF #3, our final try, on 12-2-11. ET on 12/18. Transferred 3 Grade A embryos-BFFN Planning DE IVF, late March/early April- Donors ER expected to be 4/2-4/4. PAIF/SAIF welcome

    FET 6/20 : One 5AA PGS normal transferred Beta 1 6/28 = 53.9 / Beta 2 7/1 = 220 / Beta 3 7/3 = 550!

    1 U/S  6w3d - Heartbeat 119 /  2 U/S - 7w5d- Heartbeat 169 / 3 U/S (OB) 8w5d - Heartbeat still great!

  • Both of my pregnancies have resulted from the transfer of 5 day blasts after chromosomal micro-array (both FETs).  The first resulted in the live birth of my son, who is now a "terrible" but extremely adorable 2-year old!  I'm 16 weeks along with my second.  As I remember, the IVF doctors made the decision about whether it was better to do the freeze-all at 3 days or 5 days. Best of luck!
  • Most RE's follow a protocol to do a 5 day transfer if there are over a certain number of eggs that fertilize (cannot remember the number), which generally means there will be a blast to transfer at Day 5. I think the guidelines are described at the ASRM website.

  • Both of my cycles we transferred 5 day blasts.  They were hatching by the time they were transferred.

    I had BFP the first cycle, but ended up ectopic.  I'm almost 5 weeks now so fingers crossed.

    Ditto for me. Both IVF's were 3dt since I didn't have a lot of embies. My 2nd IVF resulted in my twins.

  • I'd highly recommend doing CCS testing so you won't waste time on abnormal embryos that will likely end in m/c.  If I hadn't tested, I'd have been given a visually beautiful embie at transfer that was actually genetically defective.
