I just did 1/3 milk 2/3 formula for a few days then 50/50 for a few days and then 100% milk....course then we realized DS had a milk adversion so now he only drinks almond milk but that is how we did it.
We basically did the same thing as the poster about. First 1/3 milk, 2/3 formula, then 1/2 milk, 1/2 formula. Even though the formula was milk based, once we started giving them 100% milk, DD2 started having a really bad reaction. She would throw up (not all of it, just some) and she'd get a horrible diaper rash, to the point that she had lesions, so we tried a few others with her. She's now been drinking almond milk for 9+ months and doing great.
Both babies (we had twins) were fussy during the transition, but things went back to normal after 2-3 weeks. Good luck!

DS was taking 7 oz bottles when we started the transition so we started with an oz or so of milk and then rest formula for three days and then increased by an oz ever couple of days until he was all milk. We had a lot of RTF formula, so that is why we stretched it out so long as well as the fact that I am slightly lactose intolerant, and wanted to make sure DS adjusted well to it.

He had already been exposed to dairy a good bit so I wasn't worried about how he would really handle it.
