WWYD: NT & Harmony Results

All,

Have some mixed results that are hurting my brain and would love to share and get others perspectives.

Last week we got our Harmony results back which were great. For the three trisomies, we had low risk which was 1/10,000.  The accuracy rate is 99%.  Basically the best outcome that is reported. 

This morning we got our results for the NT Scan with blood test.  The neck was 1.2mm and was fine.  The HCG portion of blood tests was fine but the PAP A was low.  With this - the NT scan results reduce my risk rate from 1/100 (my age) to 1/58 - which is obviously very scary. The doctor said the low PAP A could be driven by a) normal variation b) a marker of downs or c) a slight issue with placenta - which would just require monitoring

We are honestly not sure what to do.   

The Harmony is more accurate and says we're fine.  The NT is less accurate but shows a risk.  It is very important to us to know or feel very certain we are not facing a trisomy issue.

Our options are to a) more heavily weigh the Harmony results and move forward or b) do a CVS to confirm.  We have a CVS scheduled for Wednesday already. I go to a top hospital in NYC which has a strong reputation for CVS - the risk is 1/400 for miscarriage.

Would love to get your thoughts and how you would think about this.

IVF #1 April 2013, BFP!  EDD 1/12/14 with Boy!

  • I would just rely on the Harmony results. The NT scans are dependent on angle and interpretation. I hesitate to do any invasive testing, so I personally would not do the CVS. However, I am very conservative about that stuff. Good luck with your decision. It is so hard to know what to do.

    the risks at my hospital/doctor are exactly the same for both- 1/400 - otherwise i would def choose the one less risky

  • I would consider the CVS only if early termination is an option that you and your husband would agree on if trisomy's are found.  Sounds like you really want to know for sure, and CVS would give you the peace of mind you are seeking.
    TTC since 10/09 Me-43 DH-44 RE and testing 10/10-11/10, Recommending IVF 1/11 New RE AMA and DOR-DH low motility IVF #1.1 cancelled 3/11 due to poor response IVF #1.2 May 2011, one perfect 8-cell embryo, 3dt-BFN, IVF #2.1 Converted to IUI d/t poor response.   • I wouldn't have considered any invasive testing when my Harmony results came back as good as yours. Unfortunately these uncertain results from the NT scan are not that uncommon. I'm sorry it's causing you stress.

    this. I skipped the NT scan so I wouldn't run into this issue. Ask your doctor if harmony has ever given a false negative. I've never heard of it happening.

  • Ugh, sorry you are in this position. It can be so confusing and scary. If it were me I would go with the results of the Harmony because of the accuracy and I think it's more reliable. I would not choose invasive testing. But as pp said you are the one who has to figure out if you would trust that everything was ok going forward, or if you would need to absolutely know 100 percent for your peace of mind. I would trust the Harmony results over the scan like others have stated. But I'm sure in the back of my mind I would have doubts and worries too. Good luck, I don't envy your dilemma! Hugs!

  • I want to thank you all for your thoughts.  I tend to make up my mind quickly and getting other POVs enables me to think through the options from different perspectives which is incredibly helpful.

    I would rely on the Harmony test vs. NT.  My perinatal group won't even give you the NT results if the Harmony ones are good.  

    With my last pregnancy (before Harmony, etc. tests were available), I got a false positive with the NT scan and had to do a CVS.  Everything was fine, but that false positive really shook me.  I did the NT scan this time, but really just for the ultrasound. I had the blood results held and went with the Harmony results, which were same as yours:  1/10,000.

    I would rely on the Harmony results because they are more accurate.
