While my twins are not here yet, we have always used the same item for our two older kids. We have a temporal thermometer and just use a disinfecting wipe (like the ones used to wipe of pacis) before each use but as far as a separate brush, comb, nail clipper, etc. No. It never hurts to have a spare in the diaper bag but it, IMO, is not a requirement.
I can see if you plan on using a rectal thermometer you should have two but otherwise no.
We have two rectal thermometers but they share everything else.

We registered for 2 simply because I'm sure we'll loose some things and then I can have some items in my diaper and not have to be hunting items down constantly.
The girls spent a little of time in the NICU and they were sent home with their rectal thermometers so they each have their own of those, but we only use the temporal one and we only have one of those. About a month ago I went and bough a 10 pack of combs because we lose them so often. But honestly, they share everything. They have their own pacifiers, sippy cups, etc., but in reality it's all community property. My MIL freaks out and tries to make them only use their own cups but I just don't care and don't worry about it unless one of them is sick (but really, if one is sick the other one is getting it anyway). When they started solids we had their own bowls and spoons and kept everything separate, but that didn't last very long. If they feed themselves they get their own spoon/fork, but if we're feeding them we use one. My MIL also thinks it's gross that we give them baths together. It's really a personal preference. I figure they put every toy in their mouth anyway and they share toys, so what difference does it make? We use separate stuff when sick (or clean things we only have one of) and don't worry about it the rest of the time.

Aside from rectal thermometers, I think everything else is fine to share. My babies have their own pacifiers, but they are ALWAYS pulling them out of each other's mouths and putting them into their own mouths. I can't stop that unless I sat between them during all waking hours. Not going to happen. Especially now that one crawls.
We only have one grooming kit. I did purchase some extra combs, but that is only because DD2 is constantly snagging the twins combs for her Barbies. I cannot think of a sanitary reason for not using the same nail clippers, files, etc. I do have lots of extra bulb syringes, but that is because they were sent home from the NICU with us, and they are far preferable in my opinion to the ones that come standard in the grooming kit.
Really, they are eventually going to share everything anyway. Just yesterday during Tummy Time, Aubrey snatched Avery's pacifier out of her mouth and popped it in hers. Nevermind that they have identical Wubbanubs! I wouldn't buy an extra grooming kit, unless for some reason you really want to.
So if they don't share rectal thermometers and you have another child, older or younger whatever, would you reuse the rectal thermometer for the different aged child?
I have a rectal thermometer from DS. I washed w hot soapy water and then swabbed it w alcohol after each use. Why can't I use it again w the twins? It gets thoroughly disinfected. What am I missing?

I saved the rectal ones from the NICU and they are put away just in case the batteries run down in the temporal.
