Question about subchorionic hematoma

Hi ladies,
Newly joining this board as I just found out yesterday we are expecting twins! Or at least we were told so :. My question is that I have had some brown spotting occurring for almost two weeks and once it was bright red which triggered a trip to the hospital to get a RhoGam shot. I had a miscarriage in April and was concerned that's what was happening again. However, had an ultrasound and it showed everythin was fine and that we were 7.5 weeks along with twins although my HCG levels aren't as elevated as to indicate twins, but I know those vary widely from person to person.

I aske my doc what he felt was causing this spotting and he said perhaps just ongoing leaking from menstrual blood. But as I was readin a few other people's posts and articles I'm beginning to wonder if I have a subchorionic hematoma that's causin the bleeding? I also read that in early scans these clots can be mistaken for a second gestational sac until fetal poles and heartbeats can be detected.

I didn't have this information when I was at the doctors, I was too shocked by them saying I was carrying twins so I am goin to call the doctor back to discuss this and as I have another ultrasound next week will be sure to mention it then too. But wondered if any ladies on here had every experienced something like that? Or is light spotting with multiples more common?

Re: Question about subchorionic hematoma

  • Did the doctor not show you their heartbeats? Did you get any u/s pictures? If you were having twins, it would be pretty obvious. At 7 weeks, they would have heartbeats. I had my first u/s at 7w3d and you could clearly see all three. (A link to the video is here.)

What I'm saying is, if your doctor said you were having twins, and is a decent, intelligent doctor, then you're having twins. If, at 7w, he mistook a SCH for a baby, he's a really shitty doctor.

  • I did have a sub chorionic and the bleeding at 11 weeks that caused me to go to the ER because I thought I was miscarrying where they did an ultrasound and discovered nope not miscarrying but guess what, you're having twins! They were able to see the sub chorionic though and it wouldn't be confused at 7.5 weeks because at that point both babies heartbeats should be noticeable plus it wouldn't have been in the fetal sac, next to it I believe. It is common to have bleeding with twins though. Good luck at your next ultrasound!
  • I had a SCH at 6.5 weeks with extensive bright red bleeding, with triplets. They could see 3 heart beats at that time. I think any experienced sonographer and the radiologist would reveiw after to confirm would not mistake fetal poles etc. If they told you twins, it is likely twins. GL with you pregnancy and at your doctors appointment and congrats!
  • Well here is why it was all confusing in my opinion.  When I went to emerg, the doctor on call did an abdominal U/S but I was only 6 weeks at that point, so it didn't show much that he could see.  So he sent me for a transvaginal ultrasound 5 days later.  This ultrasound was done at the hospital too, but there was no doctor there.  The ultrasound tech was the one who did it and told me she was not allowed to let me see the screen or tell me anything about what she was seeing.  I was told I had to follow up with my doctor.

I saw my doctor yesterday and asked about the ultrasound and apparently he doesn't receive the actual ultrasound pictures, he just receives the information that the ultrasound tech wrote up about what she saw while she did it.  So since the tech had no previous knowledge of any of my history and did not know I had come in due to bleeding originally I just wasn't sure if it would have been something that could be mistaken easily? I was also told there was no mention of fetal poles or heartbeats in this write up?.  Just what I was measuring at and the dating.

Super unhelpful!  And since my doctor (who is just a family doctor and not an obgyn) doesn't have the pictures he's just basing what he's telling me off what the tech wrote because neither he nor I have seen the pictures.

My next ultrasound is on the 27th but again, no doctor is present it's just a tech that does it.  But hopefully I can get some information out of this one?  Do obgyn's do their own ultrasound instead of sending me elsewhere?  Maybe I need to be getting referred for a ob for the remainder of this pregnancy to avoid further confusion :)

Thanks for everyone's responses!  I very much appreciate it :D

  • If you are pregnant with twins which all signs right now are pointing to yes you should not only be seeing an obstetrician but a MFM high risk OB. Good luck!
  • Well, hopefully the tech knew what she was talking about! GL at your next appt!

  • My first u/s was a bit after 6 weeks and that is when we found out we had twins.  There was some light bleeding/brown spotting around week 10 but then around weeks 11/12ish (sorry, I can't remember exactly) I had a bright red bleed - then again a week later.  Both times I went to the ER and they did scans - they did not mention the SCH either times but told me to go directly to my OB the second time.  Sure enough, he was able to locate the SCH.

As PP have stated, if your doctor has told you that you are having twins I'd look into setting up and OB so that they can refer you to a MFM.  I've only been dealing with my OB up to this point - but my first MFM is next week (normal practice in my area).  I guess my point is - you need to find a doctor skilled with twins and pregnancy going forward.  Not only to give you peace of mind - but to make sure the babies stay nice and healthy.

  • I will be calling tomorrow to check on te status of te referral to the ob. One was put in prior to my pregnancy due to an abnormal pap and when I found I was pregnant again I requested a second referral be sent to the same ob regarding the pregnancy. I was told I would get a call but havnt yet. So I will follow up with that tomorrow. I had never heard of a mfm specialist until this thread but I've looked this up and we do not even have one in the small city I live in only three obs in total seems te closest one is an hour away so ill see what the process is for getting to one of them :. Thanks again ladies!

  • Kristin - our town doesn't have an MFM but one comes up weekly from the closest city - when you call about your OB ask them if they work with an MFM and/or if there is a particular day one comes to your town.
